NEED quick answers please. Sick Note AIBU ad WWYU

Went to docs on Friday afternoon and got a
7 day sick note.

Just getting it out of bag for DH to take into work (work in same place but diff depts) and it has:

My name: correct
Date of assessment: incorrect. They have put my date of birth and not date I got the sick note! So IT looks like I have 7 days off sometime in the 80s

The doc has signed it but the date box is empty under it...

AIBU to just date it myself- could I get in trouble?
Or should I call surgery tomorrow morning and ask for a new note for work?
Or should I just hand it into work and pretend
I never noticed?

I can't make my writing look like doctors because.... Well they all write awfully!

I'd attach a note to the sick note pointing out the error and asking if they'd like you / need you to get a new one!

Give a copy to work with your correction and explanation. Send original back to doctor asking for correction to be made.

I don't think I can take a copy... I'm at hospital first thing tomorrow. I might ring my manager and explain and then go to surgery after hospital appointment

I don't think you need to worry if you explain; doctor should correct it.

My dr has 2 different offices so depending if/ where they are working I may have a trek across the city lol
