Positive coeliac blood test in toddler, likely biopsy or not?

Hello

Hope I’m posting in the right place. I think my son (2) may have coeliac disease but it’s not yet confirmed. He had blood tests due to chronic constipation, no weight gain in six months, very low appetite etc. We have a family history of coeliac and autoimmune disease. His TTGa is 40.2 (cut off 14.9) so that seems positive. My concern is that he may need a biopsy because, whilst it’s positive, it’s not an outrageous positive. Does anyone have experience of this? He’s already had one general anaesthetic this year so I’m reluctant but obviously will do whatever is advised.

I thought generally in children they don’t do an endoscopy( but consider a positive blood test to be sufficient for diagnosis?

My daughter was diagnosed coeliac from a blood test when she was four - no known coeliac family members. Paediatrician said levels of blood test were sufficient without biopsy, but I can’t remember the levels I’m afraid. Sorry not to be more help, but I got the impression they veer towards avoiding biopsy when they’re little.
