3 month old baby with possible formula issues

Hi, looking for advice quite desperately.

FTM here. My son has just turned 3 months old, initially breastfed for about a month but latch was bad and my milk dried up due to stress so moved him to SMA formula. At about 6 weeks he started showing signs of wind/reflux which the doctor prescribed Carobel for and I added Colief to his milk. Eventually I switched to SMA comfort as it was much less complicated and had heard good things.

At about 8 weeks until present day he is struggling to poo and has screaming fits several times a day, clenched fists, goes red in the face and loses his breath and cannot be consoled. He’ll have a poo maybe every two days and will be okay with the odd fuss and crying for the day then next day cycle starts all over again . His poo is smelly and green and never solid. It’s been like this since about 6 weeks.

He was put on Lactulose which didn’t do much and I have since stopped it as I’m confused as to why he needs this as his poo is not hard and solid. As a last resort I’ve switched him to SMA lactose free formula which I have a gut feeling isn’t going to do anything and will give this another week or so to see. I’ve heard this is easier to digest.

Since turning 3 months he has got worse and is extremely sensitive to stimulation and groups of people particularly when he is feeling in discomfort. Bathtime used to be the only distraction and comfort for him but even this doesn’t do much lately. My baby is alert and reaching all his milestones and when he is not in pain/discomfort is a very happy baby. End of my tether and seeing my son like this is mentally and physically exhausting. Any advice please? Needless to say Drs are hopeless.

I was going to say it could be Tongue Tie until you got onto the bit about pooing. I suppose there could still be a tie?

Has CMPA been ruled out as well?
