HV thinks this might be what is wrong with dd, went to GP he agrees so has prescribed Nutrimigen. I thought silent reflux but they both think this is more likely.

How long should it take to see a difference?

It is something she'll.grow out of? I'm assuming there is no harm in trying, if it stops the screaming.

Yeah it is the cold water I don't like, it won't kill the bacteria. Can you heat it then? The staydown you have to leave to naturally rise to room temp. Heating it caused it to clump.

I am seriously considering a private paed referral.

Yes, on the tin it says something about it not being suitable for prems or babies with compromised immune systems as it is not sterile. I just ensure that every thing else is clean and that as soon as it is mixed it is refrigerated. You can warm it, it doesn't advise not too and doesn't cause it to clump. You do need to use a faster teat. I dont recall clumping with stay down either, we used it for dd2, that was about 20 months ago now though so my memory could be failing me!
I think you are right to go private. My gp phoned the paed this time around and spoke to the paed on the phone who gave him a treatment plan there and then. The paed was known for treating babies with reflux so it was great to have it dealt with so quickly.
