DLA form completion advice

Hi just completing DLA form for our ds. Q4 is any other names the child has had…. I’m presuming I need to put his pre-adoption name here but just wanted to double check this is what other people have done? Thanks!

No I didn’t, I think that’s for of the child’s name has changed eg parents got married. I ignored the question.

No. You don't need to do that for anything - passport, dla, pip, dbs check, driving licence. Adoption makes the old name disappear for admin purposes.
