Couldn't think how to word the title!

I'm trying to do DDs provisional licence application. It asks for birth surname and mother's maiden name. How am I meant to fill it in? The legal person she is now didn't have that birth surname, and my maiden name doesn't match it in any records. Has anyone done this recently and can help please?

I should remember, only did this a few years ago !
Is there a question asking if you have ever been known by another name?
I think we might have just our current names, though I've never been married so didn't have that complication
Then put his birth surname in previous names and provided adoption certificate as proof of identity

I only used adopted names, no 'known previously as' or whatever the wording is.

We already had passports in adoption name so that was the legal evidence if needed.

It asks for birth surname and mothers maiden name, there no option to not fill it in and the help button doesn't help! hopefully screen shot will appear:

I am certain I filled in adoption name as birth surname. I have never put their pre-adoption surname down on anything. I don't think there is any legal record of their birth name once they are adopted?

As I am a single adopter, I put the same surname in for Mother's maiden name as for birth surname.

I don't remember any problems with getting the provisional except for the photo- they were really picky about that!

For passports etc you dont use the birth name. This is a badly worded form. Btw this is the old system, I think. There is a newer one

This is a very interesting question and something that I am interested in.i have wondered about this when filling in official forms for my adopted children. I complete them like they were born to me and birth name is our name, mother’s maiden name is mine. However, what do people put for mother’s maiden name in the case of a child adopted by two males or a single male?

Well how I filled it in made the computer say no, so they sent a form in the post which didn't have those questions on Confused. What a shambles of a system.

I think legally, the adoption certificate replaces the original birth certificate doesn’t it? You get a new NHS number and everything. I would put adoption name on.
