Passport - name change

Would love some advice/a reminder!

DS had a passport issued in his birth name pre-placement. We now need to get him a new passport with his adopted name.

Am I looking forward a change of name form? Or something else?

The weird thing is, I went through exactly this with DD a few years ago and it was tortuous but I got there in the end. The trouble is, I think it was so bad I’ve blanked the entire memory!

Thanks for any advice.

It would be a renewal and the form would be completed with the new details. You'd need proof of the name change and a countersignature if they are under age 12.

Didn't want to read and run as we did this, but it was nearly 5 years ago and I can't remember! I 'think' we just sent off a change of name form, along with his adoption certificate? It wasn't difficult, I know that!

There isn't a change of name form. You just do a renewal of the old passport but provide documents to show the name change.

Just done this. Just a renewal passport stating new name plus had to send adoption certificate and an ‘official document’ with name on.
We used a hospital letter

@Moonopoly I’d never have thought of an official document. Can you scan it in or do you have to post it separately?

(and I’m struggling to think of a document with his name on. Maybe pre-school would do us a letter)

Do you have anything from health visitor or Dr’s? We used one with the NHS number on and this was accepted. I think it says from a Government Dept. We had to send original documents in post with adoption cert.

He’s only had one outpatient appointment, and I didn’t keep the letter 😩. Our GP do everything via an app. I’ll have a hunt around!!

The annoying thing is though that the adoption certificate is pretty conclusive! It’s weird they want something else.

When we renewed our daughter’s passport our social worker to tick new passport and NOT renewal because her previous identity no longer officially exists. This is what we did and all was well.

She's still the same person with a different name though which is why I'd do it as a renewal. Probably makes no real difference which one you tick as you still need to provide all the same paperwork
