Evening all,
I am a bit overwhelmed by the application for adoption order form I need to fill in and was wondering whether some of you may help? My SW is currently on leave so thought I'd make a start of it myself.

One of the first of many questions I need to ask is About other orders or proceedings that affect the child in the section -

m) To the best of my/our knowledge, no proceedings relating to the child (other than any placement order, freeing order, permanence order or any maintenance order as given above) have been completed or commenced in any court.

or
The following proceedings relating to the child have been completed/commenced
(in addition to any placement order, freeing order, permanence order or any maintenance order as given above)
See Note 14
Type of order made
(or applied for)
Date of order (or date of next hearing)
Name of court Case number (or serial number)

Do I need to put details of the ICO that would have been in place when the child was first removed. or just tick the first box m)

thanks in advance x

Our social worker did all this. I would wait until they're back.

hi gordongrumpy thanks for reply but we have been told we need to complete ourselves x

Who told you that? It's your social worker's job to help you, and presumably it's only a week or so leave.

It's in their interests for you to get the AO done, and off their books. They will know what needs to be said where, as they do this often. It's their job. You probably have enough on your plate.

Hi gordongrumpy, yes they will only be on leave for a short while. Obviously the SW will help once back from leave as there is a lot of information I cannot complete. But we have been told to make a start and send off the application and pay the fee one completed.

Hi there,
We started our form too, I would advise you to be cautious though as if anything is even slightly wrong the courts often send the whole lot back further delaying your application. By the time the clerk has read it and posted back this can mean weeks more. If there is anything you are a little unsure of I would leave it completely blank and ask your SW to help you complete when they are back from leave.
Congratulations on being ready to submit your application, hope all goes smoothly

Hi,

I’ve completed and submitted two adoption orders myself, as instructed by my adoption agency. Ticking the first box is fine - you don’t need to include details of the ICO. Hope that helps!
