Adoption order application - statement of fact?

Just filling in form A58 to post to the court. It asks for a statement of facts if asking to dispense with the birth parents' consent. I asked our link at the adoption agency about it and she said we didn't need to send one as LO was placed via an agency, but I can see no mention of this in the accompanying notes. Any thoughts?

I’m sorry I really can’t remember the details in the form filling, I could dig our copy out but might not be relevant as different circumstances. I hope someone comes along who does know the answer! What I do remember is how stressed I was was trying to fill the forms in! Confused good luck and I hope you get a more knowledgable answer soon! Flowers

I panicked about that bit too! We didn't send anything in re that, our LO's social worker reassured us that it was covered by what the social workers (the child's and your SW) provide to the court following your submission of the A58 form. If you're concerned you could ring the adoption case team at the court to check, I called as I wasn't 100% clear on whether I needed to send a copy or original of another doc and they were very efficient and answered straight away. You should be able to find their number if you Google the court details.

I can't remember the court paperwork, I'm sure our social worker did most of it- I literally remember the names being the only thing I contributed! So, I would certainly expect SW to guide you significantly, this is their area.

It’s worth checking as we did have to write a statement of facts. Our SW supported us to write it though and it was just about why we believed us adopting our son was in his best interests.

If I remember correctly, our SW wrote it. It was a whole other document that once your form is handed in, the court asks the SW for. I think it was referred to as 'Annex A'.
