Do you mean a registration of interests form to start the process? If so you need to decide which local authority or agency you want to adopt with and contact them. It is best to go to a few open evenings for different la or voluntary agencies in your area to see which one suits you and indeed has children that you would be a suitable adoptor for.
Why not google a few in your region and get contact details that way ? Good luck with it all
