Help with GP and records

Hi there, I am new to this forum and also just starting the adoption process with my partner. I recently had to gain a medical report from my GP for life assurance. In it I was surprised to see that some information that I felt was not quite accurate and I am worried it might look bad on the medical report that is going to be done imminently for the adoption process.

I always get a bit tongue tied when discussing things with my gp and so wondered if anyone else had to speak to theirs about their records?

As far as I know, you will need to have a standard medical (usually from your GP) as part of the adoption approval process. The adoption agency or LA will send the GP a standard form to complete, and they will fill it in while you're there usually. It is fairly thorough, and takes a while, and you usually have to pay for it. But what I'm trying to say is that this medical form is based on the examination, and you don't have to allow access to your whole medical records. Is there a GP in your practice you find easier to talk to? That might help you feel as comfortable as possible - although obviously it's still a stressful experience because you want everything to go well!

The agency/LA are not looking for medically perfect people. They have to check that there are no significant issues that would affect anyone's ability to parent a child into adulthood. It's OK to have things on your medical report, as long as they are demonstrably well-managed and successfully treated.

As a separate issue, if you feel your medical records might be misleading or inaccurate, you can ask the practice manage for an appointment to view your record, so you can read it properly and see what is there.

Hi, no experience of having to speak to GP about my records but the medical is very specific and not a provide all records kind of thing. My LA provided a questionnaire for my GP to complete, from memory I think the only open question was to list hospital stays/treatments and my GP discussed this with me whilst completing the form, he went through my notes checking that it was correct before writing it on the form.
