Cortisol

Hi. Short and sweet.
Just wondering if anyone knows anything about cortisol levels/testing? Or is it all a lot of rubbish?

Cortisol testing is very important for some serious illnesses eg Addisons disease (too little) and Cushings syndrome (to much).

You'd know if you had Cushings Syndrome as it usually diagnosed in childhood but Addisons is an autoimmune disease and is very insidious. If you are very tired, have lost weight, a craving for salt and a healthy looking tan that's not fading then see your GP.

I was diagnosed with Cushing’s at 60, it’s not just a childhood disease. Can be caused by a tumour, or using e.g. topical steroids for eczema.

The link gives good information, the path to diagnosis and treatment can be long and difficult.

So if you think you have adrenal insufficiency/Addison’s disease, it’s a really important test and will be life saving as AI can be fatal. I have adrenal insufficiency (caused by cancer treatment) and I was very ill when I was diagnosed. Like hadn’t eaten in a week, drifting in and out of consciousness sort of ill.

I do think the whole ‘adrenal fatigue’ sort of thing is a bit of wacky baloney. The only people I knew who thought they had it had really poor lifestyles. Too much drinking and drugs, too much coffee, not enough sleep or exercise. They didn’t have an issue with their cortisol. They were just treating their bodies like shit.

If you do think you have the signs of adrenal insufficiency (or on the flip side, high cortisol or Cushing’s disease), you can pursue getting tested. You need a morning cortisol blood test, drawn between 8-9am, not one of these saliva tests you see for sale online. You can have it done privately if you want but it does need to be a proper blood test one.

AI isn’t something I’d go out looking for if you are otherwise well though. It’s a long term condition that requires lifelong steroid replacement. I take steroids 4x a day and I have to carry an emergency injection with me everywhere and it makes doing all the usual things I used to take for granted really tricky. Even things like sex require carefully timed medication doses. You get used to it, but if you generally feel okay, I’d just feel grateful for that.

I should point out that I’m not sick. I’m just extremely tired and my algorithms seem to be pushing cortisol overload being responsible for a lot of what ails you.

Thank you, sorry I didn't realise that. I hope you're well x

I have adrenal insufficiency, from birth. I assure you, if you had AI you'd be very, very sick and it will eventually be fatal. Cortisol "overload" isn't really a thing.

Although I've never heard of carefully planning sex around medication doses. 🧐

I have autoimmune Addison’s Disease, and like @mindutopia I take medication 4 times a day and have to think ahead regarding my energy levels, keep life as stress-free as possible, and generally be very, very aware of my body and how I feel at all times. Unlike diabetes there is no way to check cortisol levels with a finger prick.

adrenal “fatigue” means nothing medically.

go to addidonsdisease.org.uk to get more info and if you do think it may be an option you will need to see your GP for early morning blood tests. (You would be feeling very, very ill though, not just a bit ‘off’!)

I have Addisons and I’m heartened to see some knowledgeable replies already. If you think you need your cortisol levels testing then you need to ask your doctor for an early morning (8-9am) cortisol blood test. If it comes back under 300 you need to be referred to endocrinology for a sst test (google this).

Cushings can strike at any age, as can adrenal insufficiency. There are several types of adrenal insufficiency - Addisons or primary, secondary and tertiary. I won’t bore you with it all as it’s very complex but you can google to find out more. I advise in an Addisons group.

This leaflet is a good starting point in understanding it all -

Cortisol overload is not a thing. It’s a made up term usually designed to sell useless supplements. If your adrenals are working well they will regulate themselves.

seahorsessky · 03/10/2026 23:40

Are you meaning the blood tests? Might be worth seeing the GP depending on symptoms. They will tell you about when the tests are, think it needed to be morning.
