has anybody had this issue with the nhs

where they tell you one thing verbally but say another thing in your medical records last year i was refered to hospital for an issue with eyes i was told it was to risky to do an op and i was to go back in 6 months for a review but was told they didnt believe things would get worse this wasnt what was said in my records though it was said in those that it was suggested about having an operation i was given leaflets and to read over then told to think about it then at my review i was told my condition had got worse but in records the doctor just said that my condition was stable

Are you sure you have the full set of the notes sent to your GP?

When they said too risky they meant that at that point the benefits did not outweigh the risks. If you wanted the procedure you were given the information, a review date and the information that it was likely stable. That doesn’t mean definitely, they can’t predict that.

Are you now not able to have the operation due to deterioration?

it was dr at hospital who did further tests on eye and told me it was to risky a procedure to do operation but that wasnt mentioned in records he said that it was discussed about having operation and i was to think about it thats two completly different things he also told me at first appointment eyes wouldnt get worse then at second after doing follow up tests noticed they had got worse

i understand that im not an idiot my point is saying one thing verbally and another in notes example saying at follow up appointment verbally my eye issues had got worse but saying in notes that my eye issues were stable as if there was no difference thats misleading and contradictory i also find it strange that after first visit with doctor he didnt say in notes i was advised not to have operation just that it was discussed and i was given some leaflets and it was as if i was to make a desision on those rather than what he had verbally told me

yes ive i requested my med records for the last four years i made sure that was after my eye issues had started

I think it is semantics. You were told about the risks of an operation = discussion about operation. Were you given leaflets or signposted to information? A condition can change but still be regarded as stable. When is your review appointment?

i wasnt advised about the risks i was advised not to take it as the risks were too high thats why i decided not to take op because of drs advice the condition deteriated why not say that rather than say it was stable stable gives the impression no better or worse

i agree op. i think we should be allowed to read what has been written up before we leave. so we can correct it.

i’ve found things missed before. mostly when there have been multiple things and only one or two written down.

really annoying and we have no come back in the event it goes wrong as we can say “but i told you my symptoms and fears yet it’s not recorded” and they will say “thete is no evidence that was discussed”. well how should i know that it’s not recorded. i have to go on faith. doesn't seem professional in this day and age.

i agree everybody should be able to see whats been written at appointment and i wonder how much of this goes on and going on posts on here by others i dont think people are always aware that things are left out or one things said verbally and something else gets written in your notes and if you notice afterwards your a liar or you must of mistunderstood whats been said

Yes, and it is the eye hospital being confusing. I have been seeing two different departments for two separate conditions both telling me ...nothing wrong but want to monitor you

Recently found out (last week for one dept) that I have the start of issues. Neither department seems to confer with the other one and at two appointments I have had nurses panic as my pressures were 'too high' ...I had to remind them they needed to adjust my figures. They had forgot to look at my records
