How can I complain about social worker

I’m not obliged to see a mental health social worker, it’s voluntary. Although, it was a long fight to get one in the first place. It’s been 6 months, and they haven’t done a single thing. She’s been turning up to the house occasionally, missing most appointments and not following through on anything we speak about at all. It’s been constant chasing. I sent her an email last week explaining I’m ending contact with her.
As part of the complaints process, do I need to outline every single thing she has done wrong or can I just say “not turned up for most appointments, not been contacting me to explain, and not followed through on anything we spoke about” or do I need to outline everything specifically.

I would list everything in detail. To be honest I worked professionally with social workers for fifteen years and many of them were very flaky and unreliable.

Probably more impactful by writing out is detail and in bullet points.

Did not attend meetings on date x,y,z

Agreed to do action p on date x. To date this has not been carried out.
