I need help getting rid of an assistance worker

I have been given a personal assistant through social services. My life is very chaotic and I have too much to manage myself but I have really had nothing but issues with them to the point where I need to get rid of them and wish I had never asked for help:

  1. Being so late they almost might as well have not come because they only had minutes left
  2. not contacting me and
  3. not turning up
  4. not telling me when they are next coming
  5. turning up unannounced
  6. telling me when they are next coming but getting the date wrong and then
  7. blaming it on my memory. I do have a bad memory but they skipped an entire week when they are meant to see me on a weekly basis.

I have a phobia of hospitals but need to get seen for health issues that I've been having for a long time. I also have severe agoraphobia, cant use public transport which they were made of before they agreed to work with me and I explained to them that we would need to build on it slowly by taking short trips towards the hospital, sitting outside of it and then make an appointment BUT she has ignored all of this and has made an appointment for a few weeks time and she wants to go on public transport! so I am going to have to phone the place myself and cancel it so that an appointment isn't wasted. This is going to come across that I'm not engaging with them.
I'm not good at wording things and tend to waffle on or give information that isn't relevant and I'm not sure how to let this person go.

Can you not email the people that assigned you the assistance worker and detail your issues as above

Have SS assigned this person to you? If so, contact them and say they aren’t suitable for your needs so you need someone else. Give them the reasons why just as you have detailed them above.

If you’ve hired them directly just give them a letter giving them notice saying it isn’t working out. If they press for why just tell them they aren’t taking your needs into account and you need someone who can work with you, not railroad you into doing things beyond your capabilities.
