When do teenage spots tip over into acne? How do you know?

Thank you. There seem to be more and more coming, but do settle in-between (new replace the old). Some are inflamed, but they do go, then more come. I had spots around my hairline, but not everywhere like DC.

Our DS who is 16 was getting spotty. I offered him spot cream to which he accepted and then just asked him if they were bothering him. They weren't at the time, so I told him if that changed to let us know and we'll get him to the GP.
He started with pimples and then they turned to blind, angry red bumps which I would say are acne with the risk of scarring.
He came to us to ask for us to take him to the GP. He got a benzoyl peroxide cream and some long term antibiotics, so his skin is improving.
He takes after me; all my fault!
