The first one, but look on the church website or paperwork, and do what they do.

You could always say 'The Church of St Thomas' instead.

St. Thomas' Church, London

is your best option. Definitely use Church. The apostrophe goes on the outside and you don't need an extra -s (St. Thomas's Church, London)

All acceptable options:
St Thomas' (traditional use of no second s as is Biblical mame)
St Thomas's (also grammatically correct as second s is optional
Church of St Thomas

Well, you need to check the church website what the spelling is as e.g. the church in Finsbury Park spells theirs St Thomas Church.

The old form would be St Thomas’s but St Thomas is fine especially if the church calls itself that
