Work stress

So... I have been off sick from work with stress ( work related )
As part of my return to work interview( still to happen ), my manager has asked me to complete a stress questionnaire ahead of the interview next week.
Should I be open and honest in the questionnaire/ interview 🤔 or should I gloss over everything.
I am feeling better now, I think I just needed some headspace.
I'm worried that if I am honest, there may be repercussions or that it may be suggested that I am unable to cope and that this may be documented in my records.
Any advice ?

Definitely answer honestly! It'll help them sort a return to work if that's what they're planning on, it gives them the best chance of supporting you fully to come back to work without it causing any other stress. If you've been off with stress that will be in your records anyway, the stress questionnaire isn't to add comments to your record at all - just to make sure you're okay to go back and things like that.

Don't gloss over anything, it won't help in the long run 😊
