Question for doctors

My mum was in A and E today. I think over the day we saw four different doctor teams and various nurses.

They all asked the same questions and I was wondering why. Is it to test if the patient is alert or if they’ve missed something? Or is it because the hospital IT makes it really hard for new doctors to get this info? Or another reason?

This is definitely not a criticism btw - I just wondered what was driving it.

It's better to here from the patient themselves incase the writing misses finer details.
I sometimes preface a repeat questioning of a patient with something like; I know you've been asked all this before and I've heard from X,y,z who saw you earlier but it's better to hear from you directly.
