Hi, I'm helping to organise my first Easter Sunday school lunch at my Church. Its for children only, there will be activities, crafts and games.

To the parents who have booked their child, I need to check

Any dietary requirements/restrictions?

Allergies?

Also do you consent to your child being photographed?

Have I missed anything else.

Parents already know the date, time, venue, pick up, itinerary, contact tel no.

Thanks for any help, I feel such a rookie, others are veterans and I want to use my own initiative.

I'm assuming you will have parent's contact numbers and an emergency contact number?
How long are the children there and how old are they?

Possibly a medical section as well for parents to fill in if a child has asthma or another medical condition you may need to know about. Can't think of anything else you might need. If they are school age I'd assume help with toileting etc. is not going to be an issue.

Gdpr: what you will do with their data and how long you will hold it. (School tell us each time)

If the children have any disabilities/conditions etc.
