Anyone good with consumer rights?

I have a phone with Vodafone. They have a rewards section that give away stuff or give discounts. I got a set of cordless hair straighteners however, I have had problems charging them and turning them on (I have to use a bit of force with the on off switch and sometimes they come on, sometimes they don’t). Vodafone told me to contact the supplier which I did and they’ve asked me to complete a form and they will then arrange a return but I don’t like the wording in the form. It says

In signing this document, I confirm that the item being returned are faulty and were so on arrival in accordance with the activate in demand returns policy.

I understand that the faulty item will be checked on arrival at their distribution centre and that if the item is not faulty, I understand that I will be ineligible for a refund and the item will be disposed of.

It also says the return must meet their return conditions which state that if more than 14 days have gone since I purchased the item they cannot offer a refund or an exchange. I ordered them on 4 November.

has anyone come across this before and can offer any advice on my rights.
