3 young girls died at a MH facility - why no arrests/prosecution?

Time and time again shit like this happens and people "apologise" - how can that be it? These girls were in hospital together, surely the staff involved in their abuse contributed to their deaths? Corporate manslaughter? Individual staff prosecutions? How can this just go on and on?

I can't help but feel that NHS employees are rarely held accountable for their actions.
