Had my review at work today. I've been slack and left things to the last minute/not contributed when I should have. Was expecting a bit of a rollocking today, or at the very least and uncomfortable conversation about my performance. However, my boss was fine with me, No issues over my performance what so ever.
I've got off this time round, I need to up my game ahead of my next review. I have lots of issues medically (think intimate areas) and I worry like hell about these to the point I cant sleep., but am not about tell my boss about them. The anxiety and worry affects my performance/frame of mind.

Well take it as a good thing that even though you may think you haven't done your best, that's not showing up in your work. And you know you can do things better this time round if you have been given a second chance (in your own head) so you won't have that horrible anxious feeling.

Just popping on to say I'm glad that this work stress has at least been taken away from you.

I'm in a similar situation, currently trying working on a project at the other end of the country but awaiting a colposcopy for bleeding after sex, my head is a mess so I totally sympathise with what you have been going through.

Its great that you feel you have been given a chance now to up your game, I'm sure you will grab it with both hands. However, if health concerns are really effecting you it may be an idea to speak to your manager or somebody else supportive at work just so that they are aware.

Google quiet quitting..

But if you think you won’t get away with it twice, get some CBT to help manage your thinking processes and work alongside stress
