I'm filling out a divorce application online, it's asking if I changed my name and I'm not sure what to put, as if I answer yes, it asks how I changed it, by sending off marriage certificate, by deed poll or other, and to provide documentation. I didn't know I had to officially change it, I just started using ex's name. What should I answer here?
