SAR Request to health centre-doesnt give me option to see who accessed my files

I think a family member has accessed my file while working at our health centre. (NHS Scot) Finally got the balls to check and have got the SAR form to access my records.
But the SAR form only has 2 options 1-applying to see medical records only or option 2-applying for copies of medical records for a specific period.
Nowhere does it give the option to see who has accessed my records, when they saw it, what they saw and their purpose for seeing it. Which I believe I'm permitted to see.
Anyone else had issues accessing their files? I aim to speak to the practice manager when showing my ID but I thought this would be on the SAR form?

Perhaps contact the manager at the health centre and raise your concerns with them.

I'm in England so things might be different. If there was a list of the times when your medical records were accessed it would be a mile long surely? Every time you had an appt anywhere within the GP surgery or a Hosp there would be lots of people viewing your records for lots of different reasons. No body should have access to your information unless they need such access as part of their job.

Sorry just reread your post. I agree, speak to the practice manager about this concern.
