Could anybody help me with a Spanish translation?

I have a highly anxious child who is unvaccinated (covid). They are diagnosed with other mental health conditions too.

I will be arranging a PCR for them and fill in the Spanish online form but is likely to have to go through a separate queue at the other end and may well be very distressed (I have warned them).

I'd like to have something on paper that says they are medically unwell and need to stay with me or just for them to be 'gentler' with them.

They will wear their hidden disability lanyard but these have been a little diluted due to the mask exemption use.

Could anyone help me compose something in Spanish that will explain the situation.
