If a child is travelling abroad with grandparent do they need a note from parents to confirm its OK? Different surnames etc

Yes. It's a bit hit or miss whether they'll need it. When I travel with DD (different surnames) they tend to only want it on way back into the country but send a note so it's not an issue- loads of templates online

And I’d check if there are any official version of that document in the country they are going to (or in the U.K.?)

Even when travelling with my own dcs, with the same surname, I’ve had some checks done when going through security. Nothing major (they asked the dcs who I was) but I know some people who had more problems.
