Baby passport -Asked to confirm identity again

Has anyone had this ?

I had someone confirm baby identity almost 5 weeks ago (same person that did my other children) they confirmed this person had provided details and was told to send documents , did this and now today I had a notification telling me to ask someone to confirm identity with no other details.

Should I use the same person or find someone else ?

Any idea why this happens ?

Yes, I had this back in March. Just asked someone else to do the identity confirmation again and the application was then approved the same day. Received the passport a week later.
