Wording for Medical Consent Letter

My mother will be looking after my two DC and my nephew for 3 days while we go to a wedding in Scotland. Does anyone have a template for the wording for me to prepare a consent letter for any medical treatment that may be required while we are 4 hours away?

I have tried googling but getting american stuff.

Could you ask your doctors surgery what sort of thing they would be happy to accept, then work from there?
