Refreshing to see this on a recent survey. I’m fairly sure I did the same survey last year and the question referenced “assigned at birth”.

I fill this in by saying I am Greygender demi bio on Mondays and Fridays, a masc presenting femme boy on Tuesdays and Thursdays, a omnigender or Casiogender on Wednesdays depending on the date and agender on weekends. I like to show them how seriously I take their questions….

"Sex" would be the ideal in the absence of all the other guff.

My issue with "registered" is that it is fallible, and is one of the areas where intersex people can find themselves with complexities. For example, what utility does the registration at birth have if it was subsequently found to have been incorrect?

"Sex at birth" or "at conception" is more evidence-based, it's just what someone is, but I take the point of another poster that it could be twisted to mean that sex can subsequently change.
