How do teachers, health professionals, doctors, therapists, surgeons not see they are grifting for money and kudos and not see reality and truth?

Two things stood out for me, from the Gender Clinician blog:
As a clinician or as a service, you give them what they want. Everybody’s happy. You can feel quite righteous, I suppose,
and
when I work in the psychotherapy service, that’s where I think. And when I come to the gender clinic, that’s where I don’t think. I just do what’s asked of me.”
Remember (I think we all can) that job you hated, really f*ing hated, but you got to that point in the job where you could roll out of bed, drag yourself into work, turn on the autopilot, and get through the day by thinking about the bills you were going to be able to pay, the clothes you could buy, and the lovely things you'd be able to do that weekend (not AT WORK)?
And maybe there's a nice, small team of you who all hate your jobs, but you keep each other going, and nobody can afford to quit, so you turn your brain off and make the best of it?
I'll bet we've all had jobs like that, and we can do it for quite a while before the stress just starts to wear us down.
I imagine it must have been something similar for a lot of those people who knew something felt off, but if you have to get through your day, you just turn off the warning signs.
But, I never had the power to ruin someone else's life except for firing them for being incompetent. It must take a huge switch to turn off the "caring" if you are a medic of any kind. Some medics (and we've seen the evidence in employment tribunals) seem to be able to access that switch more easily than other people.
Just switch off the brain, do as you're told, take your salary, keep your job. We really only hear about those who lost their jobs.
