Not sure if this is any help regarding moving up versus staying on the lowest dose, but I’ll share just in case.
My husband was the first person I knew to go on Mounjaro (18 months ago). He was well into the obese BMI category and hadn’t been able to stick with weight loss for more than six weeks at a time for years. Our local family-run pharmacy started offering Mounjaro - initially with an in-person assessment that lasted almost an hour with the lovely pharmacist, followed by monthly in-person check-ups where his weight and blood pressure were monitored.
He lost the weight (and has kept over 80% of it off). While there were some months when he repeated a dose, for over 75% of the time he could really feel it wearing off and knew, without doubt, that he needed to move up to the next dose. His check-up was usually just a couple of days before his new pen was due, so he could leave the decision until about 48 hours before needing it. That flexibility was helpful, as he didn’t have to second-guess in advance. He went right up to the highest dose and then did a couple of months on a lower dose and then stopped.
In hindsight, it was very expensive compared to buying online, but at the time it felt necessary, as there was much less information available to us. We’d never heard of the “golden dose,” so he disposed of each pen after four doses.
I can only afford and justify the cost now for myself because of online prices, but having seen him go through it (even though I wasn’t really involved) gave me confidence.
What has really surprised us both is that 2.5 mg is still enough for me after 18 injections. It really shows how differently it affects different people. I truly think that if you feel it wearing off and it’s no longer working well, then you probably need to increase the dose, as he did. If it’s working fine, then great - stay where you are. It’s clearly very individual, and that’s okay.
