I used to work with people who investigated air accidents. They were adamant that other there is never a single cause it's always multiple failures usually things that would not be catastrophic on their own. Even things like bird strikes are in combination with something else. And there is nearly always a human factor - e.g. a distraction at a key moment, confusion about what an alarm signifies whrte there could be multiple causes.

This is an interesting podcast on a particular air crash and the Swiss Cheese model.

Cautionary Tales – The Vigilante and the Air Traffic Controller

Air traffic controllers are meant to stop aircraft flying into one another… and if they fail, computer systems are installed to warn pilots of a coming collision. But sometimes these humans a…

From NYT just now:

Shortly Before Fatal Crash, Jet Pilot Was Asked to Change Runways
The plane had been cleared to land on Reagan National Airport’s main runway. But in the final moments of the flight, air traffic control asked the pilot to land on a separate, intersecting runway.

Shortly before an American Airlines regional jet and an Army helicopter collided near Reagan National Airport on Wednesday night, the plane’s pilots were asked to pivot its landing route from one runway to another, according to a person briefed on the event and conversation overheard on audio recordings of conversations that occurred between an air traffic controller and the pilots.

American Airlines Flight 5342, which was en route to Washington from Wichita, Kan., had originally been cleared by the National Airport traffic control tower to land on the airport’s main runway, called Runway 1, the person briefed on the events and the audio recordings revealed. But in the final moments of the flight, this person and these recordings also showed, the jet pilot was asked by air traffic control to instead make a circle landing on a separate, intersecting runway, Runway 33.

That decision, according to the person who was briefed on the event and two other people who are familiar with the airport’s air traffic, happens routinely when regional jets like the American Airlines aircraft are involved, and may have been made to help keep air traffic moving efficiently by not clogging the main runway. It nonetheless raised questions within the Federal Aviation Administration on Thursday morning about congestion at National Airport, one of the nation’s busiest, the person briefed on the event added.

It is also raising questions about the safety of using intersecting runways, the person briefed on the event said, which the F.A.A. has sought to eliminate or close in recent years in places like Chicago and Dallas because of concerns about congestion on the ground.

In case anyone wants to read about what he said. I couldn't bring myself to actually watch him. It was the same in his first term.

He's a disgusting piece of shit. But we all knew this. And Vance, Hegseth, and Duffy are dick sucking toadies.

If there is an investigation chances are the result will be untrustworthy. I really feel for the investigators. Assuming they have ethics and haven't been cherrypicked, they are going to be pressured and threatened with firing to make the result fit the narrative. If that doesn't work, the report will be heavily 'edited'.

Trump jumps right to the blame game over DC midair collision that left dozens dead | CNN Politics

And Trump presumably wants to cut staff funding even more. Wonder if he'll now change his tune on this one?

More from NYT:

Like most of the country’s air traffic control facilities, the tower at Reagan airport has been understaffed for years. The tower there was nearly a third below targeted staff levels, with 19 fully certified controllers as of September 2023, according to the most recent Air Traffic Controller Workforce Plan, an annual report to Congress that contains target and actual staffing levels. The targets set by the F.A.A. and the controllers’ union call for 30.
The shortage — caused by years of employee turnover and tight budgets, among other factors — has forced many controllers to work up to six days a week and 10 hours a day.

I can.

However, it will interesting to see all Americans stopping flying until Donald Trump tells them it's safe again.

I wonder what the airlines are saying ?

He wouldn’t know decency if it smacked him in the face

RafaistheKingofClay · 30/01/2025 19:32

It’s almost worth watching for the bit about the Russians. He starts talking about the families in Wichita, Washington DC, the USA and in Russia. Then talks about how talented the Russians killed on the plane were and says he’s very very sorry about that. None of the other nationalities on board get mentioned. Then when he’s asked if he’s spoken to Putin he says ‘no, not about that.’

I’m guessing he means he’s talking to Putin about Ukraine but it’s Trump so who the fuck knows.

Eyesopenwideawake · 30/01/2025 19:39

I listened to the whole, disgusting, deranged diatribe - just unbelievable. Yet within 30 minutes there were people ringing in to LBC to agree with him. Just un-fucking-believable.

RafaistheKingofClay · 30/01/2025 19:45

I’m trying to remember which podcast it was where the guys were talking about Biden’s inability to get the mood right with his speech after the Trump shooting. And how if you can’t get it right at a time like that then you are done. Those guys have some explaining to do.

many many years ago I was at a taster day at the local airport authority whohad a recruitment drive for air controllers.
pay was good, not excellent.
shift were shit, often split shifts of max 6 hours divided in 3x2 hours.
average employment was something short like 6 years (might have been less) and career progression limited.
it's a tough, pressured job at the best of times. properly traumatic if things go wrong.

The ATCs in that tower must feel awful today. And the Trump goes and does that.

And it’s not like the MAGAs are calm and rational beings once Trump has identified an enemy. I was watching it thinking he might as well have just stuck a target on their back.

That man is Satan personified. I don't believe in God, but I think Trump is an evil demon.
My husband was in aviation, he's now retired but is younger than one old aviation pundit we saw punditing about this accident on Australian Sky news. He (husband) is of the opinion that there is absolutely no way of knowing what happened in any aviation accident until a thorough investigation by experts has been carried out, which takes months, if not years. You do not jump to conclusions or to hasty blame in the minutes or hours after the accident. So actually, Trump is not only the Devil, he is also an ignorant, dangerous, idiot (but you already knew that).
RIP those who died, and kind thoughts to their families and friends.

From what I have heard the helicopter did not reply
twice
there is a chance that something was going pear shaped in their cabin that caused them to not look around

the Russians were the coaches of the US skating team and have lived in the US for a long time

Although as I said above, I do not believe in God, John Pavlovitz is one Christian whose ethics I admire. Today's article is worth a read/listen, I think.

The fact that most of the headlines I’ve seen are along the lines of ‘Trump links DEI to plane crash’ and not ‘Trump goes on insane ramble while bodies are still being pulled out of the water’ seems like a good illustration of how we got to this point.

That would seem to be the most reasonable explanation. It sort of got mentioned mid ramble about the helicopter but then moved back to DEI.

It’s taken a while his banging on about being a clear night and vision means they were flying using VFR.

Yeah. The sanewashing by the media is extraordinary. Especially considering he seems to be descending further into madness on an hourly basis.

user22446688 · 31/01/2025 00:25

By the way, did anyone else see this? The new meritocracy at work, I guess. I saw some speculation that Vance might be trying to defuse the potential of Tucker challenging him for the Republican nomination in 2028.

Just in case people were thinking it can't get worse...

I feel so sorry for the ATCs. Firstly they have to deal with the trauma of being on duty when this awful accident happened, and now have to cope with the orange bag of porridge blaming them.

I loathe Trump for politicising this before even the bodies were recovered, how can anyone think this is normal? It's barbaric. And signing that EO whilst sat there looking completely vacant with no idea what he is signing was just too much

heldinadream · 31/01/2025 04:58

I'd like to see every single ATC and associated worker in the US down tools right now in protest.
And I think the word 'sanewashing' that someone uses above is really important. That's exactly what we're seeing, day after day, he's reported on as if his words and actions can be somehow evaluated as within a spectrum of credible and understandable, when actually as the last few hours demonstrated he's just completely out of control and has no grip on the requirements of the job.
Surely this could be a tipping point?

Efacsen · 31/01/2025 08:18

This is all that needed to be said at this stage

“Our hearts break for the families who lost loved ones in the tragic plane and helicopter crash at DCA,” former president Barack Obama, wrote in a post on X.

“Michelle and I send our prayers and condolences to everyone who is mourning today, and we’re grateful to the first responders who are doing everything they can to help under extremely difficult circumstances.”

Plus a 'no stone will be left unturned' -type statement wrt the investigation - more than likely provided by his speech-writers as witnessed at the beginning of the press conference

Less than half the victims bodies have been recovered - so so disrespectful and needlessly cruel to the victims family and friends to say anything more

What is the purpose of this order? Does it mean anything?
Why didn't he wait for the air accident investigations to be completed?

(Or atleast wait until all the victims had been recovered.)

countrygirl99 · 31/01/2025 08:44

@notonthestairs facts are for wimps apparently.
