I was chatting to a colleague yesterday who has had their own significant weight loss journey. I was saying that although I recognise that I still have plenty of weight to lose (BMI 30), I'm not feeling as much urgency about it. It's like now that I look (and feel) 'normal', as in you wouldn't look at me and think I am obese, the level of motivation to lose weight has dropped. I still want to lose more, but I'm feeling much calmer about it. I've stopped calorie counting but I'm still eating about the same so hopefully will still lose weight, but I'm also not feeling the need to weigh daily so I don't really know, and it feels quite nice!

It's also interesting to note that although my colleague is now very thin (BMI 18 maintained for over a year), she still seems to have a degree of psychological impact from years of being obese, it seems that takes longer to go than the weight.

I've lost 10st 7lb and am now a normal BMI - it took me two and a half years but it was worth it.

I’ve just updated my spreadsheet - because the trackers on Nutracheck, Shotsy and Happy Scale are clearly not enough - and I realised I’ve hit my 10th goal before my 9th!

Adjusted for age I presume, my ‘within healthy BMI’ range has altered and now comes before my 8 stone loss!

Slightly odd but I’ll go with it.

Thinnest I have been in over 50 years or ever as an adult. I keep pinching myself (metaphorically). Still cant quite believe it.

On the jewellery, I'm noticing the big difference in necklaces, this one was practically a choker a year ago, really uncomfortable and I stopped wearing it, because I felt like it was garrotting me, now.......

I’ve really noticed this too! A couple of my very short necklaces are now perfect and a couple that I wore to fit inside a V-neck are now too long.

Hi everyone, coming back after a few weeks - great to hear the good news of the new jobs, smaller jewellery and watch bands, and non-overweight BMIs! I’m trucking along at an average weekly loss of just over 0.5kg, on 5mg.

Today I had my 6-month-after-starting-MJ diabetic review with the clinical pharmacist and I’ve come out of it worried. I’m prescribed MJ for t2 diabetes. My recent bloods are great - so great that she described my blood sugar as “over-controlled” as my “target” to have come down to is a fair bit higher than where I now am (I’m at 43, target was 58, before mj I was in the 90s so had very poor control). She said she was going to speak to a colleague and ring me back tomorrow about whether to come off or reduce a medication (my other medications for diabetes are metformin and dapagaflozin, the latter i will definitely need because it protects my kidneys) and she suggested that mj would be the one that’s removed, not metformin. But… my bloods are well-controlled BECAUSE of it! BECAUSE! If I stop it, they will no longer be well-controlled! This is a chronic progressive condition that I have. Nobody in their right mind would suggest that as, say, my blood pressure is well-controlled, I should come off my blood pressure meds. I can’t quite believe that something can be so obviously effective for my health and then removed.

if “over” control is a concern, why not remove metformin which as I know from my medical history has a far more minimal effect on my blood sugar than metformin and so it doesn’t actually get me anywhere near good control.

I did say that if that was their conclusion I would expect their support in enabling me to have a private prescription as I don’t mind paying for it and then I can go up the doses if I want to speed up weight loss, but the kidney specialist described me as the patient for whom these drugs were created and that I should be receiving them as part of my primary care.

I hope I don’t end up in the worst of all worlds of not getting it for my T2D, and then having issues getting it privately because (as actually happened when I originally I looked into it privately) the commercial provider was not prepared to prescribe to a person with diabetes.

On jewellery I had to buy some shorter chains for my necklaces a year in too! Id love some different jewellery now I'm wearing different clothes but I find it so hard to make decisions and on a low budget get anxious I'll get it wrong.

I've lost 1.75kg since mid.may so over 3 months!!! And nothing at all this last month despite going up from 9.6 to 10 and then 11 ...

I am thinking of trying 12.

I am so so grateful overall but struggling with the stall and the fear Im not going to get to the last hurdle and the fear that I wont be able to afford maintenance if 11 is basically maintenance.

Hoping I'll come out of this feeling. As where im at is already beyond lifechanging, so it seems silky to say its making me feel low, but struggling with paying so much for it to have stalled a few months and not too sure how to come out of it!

The exercise is going well though.

I'm back in my fairly full on job next week and feeling a bit anxious about that too. I'm so so tired at the moment so worried going up a dose will make me more tired .

I've had sulphur burps again. Grrrr. Pepto works but youre not supposed to take that every day.

Ive got so much to do and sort before kids go back to school and feeling so very tired and unable to sort things today.

I hear you @FatsiaJaponicaInTheGarden your body going into maintenance when you don’t want it to is so difficult when it’s so expensive! I’m on 6 now and still going up a click a week but it’s making no difference. I feel like I’m injecting water at the moment.

Actually with that said, I AM going to go every 6 days because by the weekend (day 6 and 7) I could eat like I’m not on mj and I’d like to stop that.

@PositiveAttitude ..Im Just reading your comment on page 10 ,I'm a bit behind..that's a good idea ,I wouldn't even know his to do that ..how do you get the correct amount out of the pen though
@AmberLime .your doing the same as @PositiveAttitude ..how are you working out how to do that ..it is definitely making a difference not having the fifth dose in the pen ..it is feeling very expensive..I'm literally just maintaining and paying nearly £280 to do so

@FatsiaJaponicaInTheGarden ,hope your feeling less tired and had a good night's sleep last night x I remember the days of getting 4 lots of uniforms ready and 4 PE kits ,and everything else they all needed for school ,that I knew they would probably loose ASAP.. especially jumpers..mine could never remember to bring home jumpers .
@PearlsTeapot ..I've definitely done 6 days in the past , especially on hungrier weeks.but since I lost the 5 th dose ,I'm trying to go longer between jabbing ,so this week I jabbed after 11 days .. which is the longest I've ever gone between doses ..I'm on 10 mg currently

All caught up with everyone 😍
I seem to be stuck again..but I never lost regularly..I seem to constantly have long periods at the same weight ,then suddenly it shifts a bit .
This year ,I lost 2 stone from January to now ,and so I expect I will lose another 2 stone from now to Christmas..
Compared to last year where I lost 10 stone in one year ..this does feel slow
But I'm just thankful I'm not putting it back on .
Currently on 10 mg .might try to come down a dose next month,I don't know what the lower dose than 10 is ..7.5 ?? ..I will see when it's time to order.
I'm finding it hard to find the money each month,hence going down doses a bit .
At so e point I'm going to have to part ways with mj and say goodbye..I expect around Christmas
So I think that will leave me trying to loose the last stone alone ..I don't know ..maybe that doesn't make sense to do that ..I will see the state of the finances at Christmas

@Springflowers2 10 stone in a year is incredible! I’m sure it’s quite normal for it to then slow down. 7.5mg is the dose under 10 but I think people do choose to do something inbetween by counting clicks / spacing out doses differently.

I’ve had a message back from the dr - it says the plan is to continue on the current medication and then in 3 months another blood test followed by another review. So that’s good news for now but we’ll see whether we have the same conversation again in November. Maybe as the pills develop and will presumably be cheaper than the injections then the NHS will be as happy to continue dispensing this for chronic conditions as they are with other medication.

If you buy a 7.5mg pen you'll only get 4 doses from it.

7.5mg from a 10mg pen is 0.45ml (or 45 clicks).

Like blood pressure meds ,you don't get taken of them when your blood pressure is under control..it's all money though

One of my friends was on mj for all of last year ,she lost two stone ,but had to come of it for financial reasons
So she went to her doctor and said ..I'm putting weight back on ,,can I get the jab on NHS .
Doctor said to her ..that she currently has 2000 people on her books who qualify under NHS to have monjroro prescribed
Then she said ...she has permission to prescribe it to 20 people...
And then doctor said ,that my friend definitely would not be one of them .
Which I thought was shocking...I thought if you met the criteria you got it prescribed..
In my area at least it seems you do not

@Springflowers2 your GP might mean that she has 2000 people who meet the MHRA prescribing criteria (BMI 30 plus, or BMI 27 with a weight related health condition, or the same with BMI adjusted for ethnicity) but only 20 meet the stricter NHS criteria of BMI 40 plus with 4 out of 5 of the specific weight related health conditions needed. That would make more sense to me than the GP saying she had 2000 patients with BMI 40+ and 4 out of 5 weight related health conditions.

@Springflowers2 it sounds like you don't know about click counting. Here is my analogy:

You buy a bottle of squash and make two jugs of squash: one weakly mixed and one super strong. Say you used 100ml of squash to make the strong jug and 25ml of squash to make the weak jug. Then topped both up with water to fill the two 1000ml jug.

You now have two 1000ml jugs of diluted squash - Two jugs of the same size. Mounjaro pens are the same. Tirzepatide is the active ingredient in Mounjaro (its like the neat squash). Its diluted by different amounts.

Back to out jugs of dilute squash. If you take two small cup that are the same size, and take one small cup of diluted squash from each jug, the cup from the strong jug delivers a lot more flavor (medicine) than the weak one. But the neat squash is identical in both, its just there is more of it in the strong jug.

One dose of Mounjaro is 60ml. Think of it as the small cup above.

  • A 5mg Mounjaro pen contains 5mg of tirzepatide in 60ml.
  • A 10mg Mounjaro pen contains 10mg of tirzepatide in the same sized 60ml (like the same sized cups of diluted squash).

If you wanted to, you could take a part-dose from a Mounjaro pen. For example:

  • You could take a half-dose (30ml instead if 60ml) from a 10mg pen and that would contain 5mg of tirzepatide. Its exactly the same as the amount of tirzepatide in a full-dose (60ml) from a 5mg pen. Its no different. Or
  • You could take a one-third-dose (20ml instead of 60ml) from a 15mg pen and that would also contain 5mg of tirzepatide. Its exactly the same as the amount of tirzepatide in a full-dose (60ml) from a 5mg pen, or a half-dose (30ml) from a 10mg pen.

This is called microdosing. Loads of people do it.

You microdose using a process called click counting.

When you turn the dial on a Mounjaro pen to get your dose, you will hear clicks. You will also see those clicks as lines on the dial. Should you wish to count those clicks (or lines) you'd find that a full dose is 60 clicks. One click is 1ml.

  • 1 click is 1ml
  • 5 clicks is 5ml
  • 10 clicks is 10ml
  • 15 clicks is 15ml (a quarter of a dose)
  • 20 clicks is 20ml (a third of a dose)
  • 30 clicks is 30ml (half of a dose)
  • 40 clicks is 40ml (two-thirds of a dose)
  • 45 clicks is 45ml (three-quarters of a dose)
  • 60 clicks is 60ml (this is a full dose)

And so on!

You then just need to know a bit about fractions (or Google for a Mounjaro click chart).

Mounjaro is like the cups of diluted squash. The mg referred to in the doses is actually how much tirzepatide (the active ingredient) that is delivered. So if you want 5mg - you could get a 15mg pen and take weekly doses of 20 clicks to get 5mg. This would mean that instead of taking 4 × 15mg weekly 60-click jabs, you'd get 12 × 5mg weekly 20-click jabs.

So while the 15mg pen costs more than 5mg, it lasts for 12 weeks. In 12 week you would use three 5mg pens. One 15mg pen is cheaper than three 5mg pens, so it is much more cost-effective.
