We’ve filled another thread already, with fantastic support as most of us reach week 3 or 4 and think about pen 2! Thread 3 now as we enjoy the unexpected early summer and move into our second month of Mounjaro, lighter in both scales and mood.

Hi everyone, checking in for thread 3! I'm on W4D2 today and wondered what decision everyone has made on the next dose? I've got needles for the golden dose but I'm really uncertain as to whether to increase or stay at 2.5mg. I'm wary about going too high and then becoming completely reliant.
For reference, I'm 5ft 6.
SW - 13st 4lb BMI - 30
CW - 12st 5lb BMI - 28

What would you do?
Thanks in advance for any advice!

You've obviously had good losses so far - well done! It's more about how you're feeling with the 2.5 so far and whether you need to move up? Are you still getting really good suppression or are you having to work a bit harder now?

AdotHam · 19/05/2025 21:09

So sorry @Ricoletti really hoping that you'll be on the mend soon 💜

.. I was so smug this morning and I feel absolutely wretched this evening.

sick as a dog and in bed with rennies and a heat pad on my tummy.

I hope this is just a blip and not a sign of 5ml ville things to come …

Oh no, hope you feel better soon.
I'm nervous about the move to 5mg, I've been so lucky with no unpleasant SEs so far, but have read s few others that have got them at 5mg 😬

Hope you feel better soon @Ricoletti
Panicking now that I'll have awful SE on 5mg as so far I've not had hardly any...

Can someone more knowledgeable tell me does pen stop working after number of clicks (240) or after 4 uses? If number of clicks then I might try 3.75mg and see how I go. Apologies if this has been asked elsewhere (thread moves so quickly)

dozywelshbird · 19/05/2025 22:27

Oh and well done on the loss @Pandimoanymum 😀

Sheldon4k · 19/05/2025 22:36

@dozywelshbirdthere has been a confirmed 'number of clicks pen' and a confirmed '4 doses pen'.
That i have seen on here anyway.

Im sure there will be more testing over the next few days. I am going to go with 3.75mg to start with and hope for 5 doses from the pen (using clicks). Then will go up to 5mg following that if need be.

However I have bought syringes so I don't waste medicine if it does lock after 4 doses.

Thanks @ellieinfrance - it's been a bit up and down to be honest! Week 1 and 3 were great, week 2 and 4 (so far) suppression seems minimal. I've been hungry at mealtimes throughout but have been sticking to 1200-1500 cals a day and only eating snacks I've planned in.
I haven't had any of the extreme suppression that some others have but it's been enough to be consciously making changes to the way I eat, rather than skipping any meals.
My clothes don't really feel much different and I don't think I look much different either, so I'm tempted by 5mg for better suppression and maybe more noticeable results. Then again, I don't know if I could cope with constipation more intense than this!

Coventree · 20/05/2025 07:15

Oh no! How are you feeling this morning @Ricoletti? A bit better, I hope 🤞

I am a little nervous in amongst the excitement for going up to 5mg and potential SEs. I think I might have had some vague effects from the 3.75mg but I’ve also started my period so a bit difficult to separate out any headaches/very mild nausea from that. Still no discernible change on suppression or food noise, although I’ve overhauled my diet since Easter and am continuing to lose a bit weight each week, which is great but I don’t feel that MJ is contributing much to that. Or maybe it is, silently, in the background 🤷‍♀️.

But the loses mean that I’ve tipped down into one-derland! 199lbs (14st 3lbs)🎉🎉…next goal is to move down from 90kg to into the 80s (someone else here also mentioned flipping between weighing systems to get milestones!) and then back to stones and lbs to get into the 13s 🎉

Thanks @Sheldon4k I might do the same and see what happens, so it's 45 clicks to 3.75 right?

Sheldon4k · 20/05/2025 07:24

@dozywelshbirdyes its 45 clicks on a 5mg pen.
Im going to do a super careful click count and mark the pen with a sharpie for ease next time.

Ricoletti · 20/05/2025 08:31

Morning all,

anyone had reaction at injection site? I’ve woke up today with red blotch where I injected. It’s itchy and a bit raised/inflamed. It’s about the size of a 50p coin, and is to the side of the injection mark - which makes sense as that’s where I angled the needle toward (if that makes sense) so seems to be reaction where the medicine had gone in and pooled under skin.

hasn’t happened before, i have previously had bruise or red prick mark where I injected.

went up to 5ml… new reaction as twice as much medication going in maybe?

should be worried?

Ricoletti · 20/05/2025 08:37

Sorry just to clarify, as I feel like that reads that I’m lazily asking people to google this for me 🤣

Ive looked it up and see that this can be a common reaction and should resolve in day or so… and to worry if I get hives or swelling lips , tongue etc.

but I was just wondering if anyone has had this personally happen, and how it panned out, so I can soothe my anxiety ridden brain . I trust other women ls experiences way more than a leaflet or Google in general!

Ricoletti · 20/05/2025 08:39

In other news, I’m feeling much better today after my shaky start on 5ml… sorry if my dramatic flailing alarmed those moving to 5ml soon.

I had a bit of toast then a fine sleep. Though I didn’t bounce out of bed (seems my Snow White experience was a one off sadly) , I’m not poorly and am off to work shortly x

RubyJuno · 20/05/2025 09:10

Morning All, I’ve only lost about 2lb over the last couple of weeks, 2.5 isn’t having the effects it did at first so I’ve ordered a 5 pen (ordered Sunday eve, arriving today!). I was hoping to stick with the lower dose as I jab Thursday and am away for the weekend but another 2.5 month won’t help me I don’t think. Fingers crossed for a smooth ride, I’ve had no side effects so far.

Need to start tracking calories though, I had what I thought was a perfectly sensible day (yogurt with some protein powder and coconut, smoked salmon sandwich and then chicken breast with creamed spinach with (less sensibly) a couple of glasses of wine) and it was over 1800 cals. Eeek. I’m aiming for 1500-1700, I’ve not been drinking much or eating a lot of bread but this may explain the slow loss…

About half a stone overall in 3 weeks though so I’m trying to regard that positively as a sensible rate.

Apprenante · 20/05/2025 09:40

Pleased you're feeling better today @Ricoletti . It makes complete sense that doubling a dose results in stronger side effects. I'd just keep an eye on the injection site - if it gets worse see a pharmacist. Hopefully it will just disappear.

I'm not a medical person at all . I'm just thinking what I'd do if I were you. Good luck.

Sheldon4k · 20/05/2025 10:25

My pen is due to be delivered by 1pm today, royal mail special by 1pm. However I have an emergency situation I need to attend to. If I leave a note what's the likelihood of them leaving it for me without a signature??

Im worried about it going back to the sorting office and the temp of it etc.

Anyone experienced this?

AbundanceofKatherines · 20/05/2025 10:32

Glad to hear you’re feeling better @Ricoletti. I’m sure you’ve seen this too in your Googling, but I have seen mention of taking an antihistamine for minor reactions.

I'm feeling v anxious today as my new pen hasn’t dispatched yet and I’m due to take the next dose on Thursday morning. I’m giving myself a talking to as 1) there’s still plenty of time and 2) it won’t matter if I’m a day or so late.

In other news, yesterday was W4D5 and I was RAVENOUS ALL DAY!!! This was the worst day since starting. I managed not to succumb to temptation, but it was hard and horrible.

Thank you all for being here, and for being so much better at contributing than I am.

Yorkshiregirl10 · 20/05/2025 10:38

Morning all,

For anyone wondering about delivery times with med express, I ordered yesterday afternoon and it's being packed today, due Fri. The website had a message saying it could take 7 days longer than normal.

@Sheldon4k someone had a similar issue in one of the earlier threads and asked their supplier who told them not to worry.
For my delivery, my neighbours took it in a 8am Sunday as I was away for the weekend (I was very apologetic) and they were out until the evening, this was during the heatwave and obviously they didn't put it in the fridge. When I opened the box, it was still really cold due to the freezer packs.

AbundanceofKatherines · 20/05/2025 11:28

Me again, stressing.

So, I wasn’t intending using the “golden dose” but now may have to (if my new pen doesn’t arrive).

What will I need? I have tried hard twisting, but nothing doing (probably a bit of a weakling). Can someone please link to the syringe/needles?

I’m sure I could Google, but I trust our little gang…

Zebracat · 20/05/2025 11:47

I’m wk 4 day 4 and I am also feeling quite hungry. I just had a bit more food!

PeachSnail · 20/05/2025 12:06

So I just took my first golden dose!

It went well! Much more straightforward than I had feared.

I managed to extract 0.75ml with a syringe, meaning I got a dose of around 3.125mg (from a 2.5mg pen).

Of course, it's slightly off-piste in terms of what's officially recommended, so I accept it may not be something that everyone wants to do... But here is what I did, based on advice I cobbled together from other people's YouTube vids

  • Get some Fine Micro Syringes: 32g 8mm 1ml and some pre-injection swab wipes

  • Wash your hands and have a clean surface (table / kitchen counter)

  • Swab the red membrane at the end of the pen

  • Take the protective parts off the syringe and then poke the needle into the red membrane. You can either get someone else to help you with this or just lay it on the table if you're doing it by yourself

  • Slowly pull back the plunger on the syringe to draw out the remaining liquid from the pen (I managed to get 0.75 units/ml)

  • Hold the syringe with the needle facing up and gently tap it to get the air bubbles to the surface

  • Then depress the plunger a tiny bit until some liquid comes out (removing any air)

  • Swab your intended injection site

  • Stick the needle in and slowly depress the plunger to inject the liquid. Hold in place for a count of 5 then remove the needle.

That's it!

Personally, I didn't think it hurt any more than the normal pen needle.

One watch out: it's really easy to bend the very fine needle in the syringe so be very gentle with it

I recommend this YouTube video for more detailed guidance
