Hi, just for anyone who's asking for pharmacies policies on maintenance, I've just received an email from Cloud pharmacy and this is so far the best stance I've seen anywhere. Some other pharmacies offer maintenance but Cloud seem to be the first I've seen to finally outrightly state "for life" rather than 'We'll see how it goes', 'Well have a consultation and discuss your options', 'Let's have a chat', 'You have to pay a small fee to stay on our books while you're on maintenance', 'We're happy to work with you for maintenance and we take it individually', etc. I also know they offer one month on and one month off for maintenance from what some people on reddit are on. I hope more pharmacies get on board this "for life" stance rather than beating about the bush or being limited to a certain bmi cutoff or taper.

Just wanted to post this so others can have an idea and decide for themselves as it's mostly been one or two popular pharmacies that posters constantly advertise all over the MJ forums. I use Cloud and they're excellent so far. They're on the monj.co uk list and others lists too. I'll definitely stay with them for maintenance when the time comes. No referral codes and they give discounts to their customers without you doing anything for it, so I have no skin in the game. I definitely have nothing to gain from posting, just sharing to help others.

Feel free to share other pharmacies maintenance policy on this thread if you have any in writing. It will help us all to know who to go with for support. X

I thought you might all be interested in my experience with Cloud. I have lost my weight on Wegovy as Mounjaro wasn't available when I started. I've lost 8.5 stone and want to lose another stone or so to get a BMI of 22/23 and then maintain. I'm in my 50s with a long history of yoyo dieting etc like so many of us. WLI have, I believe, saved my life and I think I may well need to stay on them long-term, if not for life.

I was really concerned about the two year limit for prescribing Wegovy so I have been talking to Cloud about switching to Mounjaro. My BMI is about 25.5 currently and as advised by them I stopped talking Wegovy four weeks ago and will need to start Mounjaro on the lowest dose.

After a full consultation, some questions and providing a full body photo and evidence of my last Wegovy prescription I have been approved and will be starting Mounjaro this weekend. Cloud have been brilliant and I am so relieved not to be cut adrift without support.
I hope this might be helpful to anyone on Wegovy who is worried about prescribing limits and maintenance.

Just for info - Numan told me I can stay on it for life. I only had a stone to lose. I was bmi 27 but pre diabetic. I’m now at bmi 22 and they said I can stay on the 2.5mg to maintain. I never increased my dose from the 2.5 as it worked for me. I don’t think I’m going to stay on it to be honest but it was interesting that they said I could. They said I could take holidays and change delivery date to suit. They said officially the only way it can be prescribed is one dose a week. They said it would be stopped if my bmi fell to less than 20.

Good to know Numan is also doing maintenance, aren't they quite expensive though?

I only stayed on 2.5 which is 229 per month. But the discount code gives £100 off and I’ve always found someone to use it! £129 is not too bad.

Thanks @Snozzlemaid my pen arrived today which is incredibly fast and efficient. I'm very impressed with them. MJ is considerably cheaper than Wegovy even at the higher doses which I don't think I'll need as I never had to go to the highest dose with Wegovy either.

I'll inject on Sunday, I wonder if I'll have side effects or if it will feel different? I shall report back!

I didn't gain any weight over the four plus weeks of WLI and that includes the Christmas and New Year period which is encouraging.

That's a disgraceful price!

It is cheaper to buy full price from other providers with no faffing about with codes.

It is £125 full price with Bolt or £129 with oushk who have a 15% discount running pretty much permanently.

I am def thinking to go cloud for maintenance As and when time comes

wonder what the diff is in the drugs I get you need to be off for 4w to get out of system before swapping

how was the 4w without any medication ? Great you didn’t gain esp over xmss

did you feel more hungry

@Blondeshavemorefun thanks, it's been life changing. The four weeks has been fine. I guess there was quite a lot in my system and I've worked really hard on learning new habits.
I did feel a bit more hungry but I think that it's good to feel hungry for your meals anyway. Not much food noise has come back and I can't eat big portions.
How are you getting on with MJ?

@DareDevil223 to echo others, what an incredible achievement and no wonder you feel Mounjaro has been life saving - I am forever grateful that we seem to be living in an era in which obesity and ill health is no longer the inevitable life sentence it once seemed to be for some of us. Like you I am in my 50s and have struggled with weight my entire life, and had always blamed myself.

This time around using WLI I have 'only' lost 2.5 stone (and need to lose one more), having lost 6 stone 4-5 years ago and then - of course - much of that slipping back on over time. I was so down about having to start the whole merry-go-round yet AGAIN, as each time the knowledge that I couldn't seem to maintain weight loss for more than 1-2 years made it seem increasingly futile. Mounjaro has changed my whole perspective on what is possible.

I have my maintenance consultation with Oushk next week and will report back. I was being slightly optimistic when I made the appointment as my weight loss in recent weeks/months has been ridiculously slow/stagnant, but I'll welcome the conversation about what the future looks like nonetheless.

Almost 4st overall. Started April sw but moved to mj and cal counting in august

still need ideally to lose 2st+ to get to healthy bmi but was almost 38 and morbidly obese 😳 and now 28 and overweight

For those who are looking for more information, I will add a few points:

Oushk currently offers 3 plans, conti, taper and flexi - the details of each are available on our website but please know that these plans are set as guides and no one has to follow them in any way.

As an overview -

Conti means continuous use of medication for as long as needed and appropriate (meaning this can be life long)

Taper means you would like to come off the medication eventually (whether this takes 6 months or 6 years, it does not matter)

Flexi is for when you decide you would like to try maintenance with lifestyle changes but would like an option to restart the medication at a later time (even at a lower BMI) - as long as you have been our patient, you will always be our patient so we will ensure you will always be looked after.

The ultimate decision about your dosage will still be with the patient and as a prescriber we will be able to help you with this during the consultation to come up with an individualised plan.

The 3 monthly "thing" is a checking point for the safety of both our prescribers and our patients to see how things are going - this is not a subscription nor a commitment. You can stop ordering at any point you like!

When prescribing at lower BMIs, its even more important for us to ensure our patients are monitored to reduce the risk of anyone slipping in to the underweight BMI.

Other bits we often get asked about:

• we will prescribe down to a BMI of 19 if appropriate for the patient.

• you can start maintenance at any BMI - even at 28 if thats what you would like to do.

• each pen still needs to be used within 30 days so if you would like to inject every 14 days then you will only be able to use 3 doses out of your pen. (for mounjaro) - wegovy is within 6 weeks of opening

• month on / month off is fine however there is no scientific data to suggest that this is a good way to maintain.

• if you stop medication and would like to restart after more than 8 weeks - you will need to start again at 2.5mg.

• if you would like to move to us from another pharmacy and your BMI is within the healthy range, then we will ask for evidence of at least last 3 prescriptions and a full body image from before you started your treatment. - this is so we can build up a case study for you on our system so we can justify prescribing for someone at a lower BMI.

• maintenance should only really start once you have reached your goal weight as this is when you increase your calories to stop losing the weight.

• if you are at a lower dose such as 5mg, you are still able to increase to 7.5mg while in maintenance if needed

• we will never "cut" anyone off but will work with you to ensure we can always put your needs and benefits first as long as the treatment is safe.

More than anything, Oushk recognises obesity as a condition that requires not just treatment, but also management and prevention AND mounjaro and wegovy are both fully licensed for treatment and management i.e. maintenance.

I had a video call with Oushk recently despite my last few pens having been from IQ Doctor. I've been prevaricating about what to do for maintenance (am not quite there yet but almost) but now that Oushk have dropped their prices and with the 15% discount, it worked out to be about £5 more expensive to order from them for a 10mg pen than to go elsewhere. And I think the only difference then being that I can nominate delivery day with Oushk which I couldn't with IQ.

So rationally I decided that was worth the £5, and even though some of Hira's advice was a bit standard, it did feel good to have the option to speak to a human prescriber for 15 mins to talk about what the future could look like. For now I'm staying on the 10mg and will think about titrating down a bit in a couple of months.

I think most/all of them would let you swap as long as you can show evidence of past prescriptions with a qualifying bmi, such as prescription confirmation emails from your past providers. I know for certain Cloud does accept at that bmi and lower still with evidence as some people have done so for maintenance.
