Congrats minerva. The doctor probably won't do anything except fill out a form or two and get the ball rolling for your exemption card.
When you have your first midwife appointment, she will normally take some history - details of cycle length and LMP date, family sickness history (diabetes, heart probs etc), and will tell you the due date that they will work to.
She will also probably advise you on stuff to avoid eating and talk about which hospital you will use.
Can't remember if you need to take a urine sample to your first appointment, but certainly can't hurt to avoid being asked to do one whilst there.
Not sure why you would be seeing a nurse, unless your practice uses them to take some of the history.
You don't need to read anything, and with access to MN there's prob very little that you need to ask .
If you haven't already been doing so, then do start taking folic acid daily though.
