I saw your title and thought, "Why not, call her what you want," but no, I think it'd be confusing. They're too similar for a brother and sister, even if they have different last names.

What do you call William? If he's known as Billy then you might get away with it, especially with different last names.

Absolutely call her Willow. It's a fabulous name. They won't be in the same room or the same house for ever and anyway, the pronunciation emphasis on both names is different with William being a longer first syllable emphasis and Willow being a longer second syllable emphasis. Please don't start me on Pork and Fork or whatever it was!

It's like Harry and Harriet or Robert and Roberta imo, far too similar although both are lovely names seperately.

Love the name and together William and Wiilow sound fine. My Aunt was called Willema bow that does sound similar. Willow is a completely different name. Go for it.
