A 14-year-old girl who was supposed to be under constant supervision at a psychiatric hospital died after being left alone by an agency worker with a false identity.
Ruth Szymankiewicz, who suffered from an eating disorder, was a patient at Huntercombe Hospital — also known as Taplow Manor — near Maidenhead in Berkshire when she was left unattended by the care worker on February 12, 2022.
The girl had been placed under strict one-to-one supervision following a self-harming incident ten days before her death, a jury inquest at Buckinghamshire coroner’s court was told on Monday.
The worker, who was using the fake name Ebo Acheampong, fled to Ghana after learning that Ruth had died following his first shift at the hospital.
Ian Wade KC, the assistant coroner, told the court that Szymankiewicz carried out an act of self-harm during the 15 minutes she was left unattended. She was found unconscious and died two days later at the John Radcliffe Hospital in Oxford.
