BMI under 27 - Off Label/less to lose

ritala · 29/03/2026 12:17

I am mid 40’s and just can’t lose the weight like I used to. I’m fed up of losing and gaming the same two pounds. I’m about a stone and a half more than my happy weight (where I would be approx bmi 23).

I have tried to find a thread for people who have been approved ‘off label’ Wegovy or Mounjaro with less to lose but couldn’t find one, so thought I’d start here. I have a BMI higher than 25 but less than 27 with a couple extra health conditions. Hoping to start after Easter. I’d like to stay with a very low dose if possible. Half a pound a week loss would be good for me. I really want to avoided side effects.

Anyone who is in a similar boat and wants to share experiences or ask questions, please join! X

@NinthBestOption I’m aiming for smack bang in the middle which I think is 21.7 (from 25.1) this was my ideal weight, I’ve been it before but find it very hard to maintain. I’m hoping to stay on 1.25mg MJ.

slartibartfastina · 23/04/2026 14:47

@NinthBestOption I am aiming for around 9.5 stone, which I was for almost all my adult life until the Covid pandemic hit along with perimenopause and a divorce! I know that’s a realistic goal for me.

SandwichesAndGingerBeer · 23/04/2026 15:06

@NinthBestOption I’m hoping to lose 7kg over a 3 month period on wegovy which will get me to 67kg and BMI of 23.5.

After the 3 months I’m hoping to lose a further 3/4kg on my own. 😬🤷🏻‍♀️

lechatnoir · 23/04/2026 15:35

Well if the Weygovy affect ever kicks in (day 9 still nothing), my target weight is 9st 10 which sounds ridiculously low compared to current 12st2 but is slap bang in the middle of healthy range. I'll be happy if I can get to 10stone 7 before the summer as this is the weight I've been for most of my 40's & 50's until recent gain and I was a very average size 12 and still have a vague modicum of boob left! .

TheLette · 23/04/2026 16:38

Does anyone know which providers I can switch to from Voy with a BMI of 27? No health conditions. For Mounjaro. Month 2 is quite expensive so wondering if I can get it cheaper by switching. Not MedExpress, they said no to me starting and I assume they would continue to say no now (plus they wasted an hour + of my life so they aren't having my money out of principle!). No complaints about Voy, they are great. I'm in week 1 still and have lost 1kg, not sure if that's the drug though versus me trying to get myself in the right mindset (I can loose weight when minded to, but I wanted some help this time round).

NinthBestOption · 23/04/2026 17:05

@TheLette you should be able to switch to any of them. They only want to know your current bmi and that you are already on wli - a photo of the box with your label or the voy prescription.

NinthBestOption · 23/04/2026 17:07

Day 1 and I feel nauseous most of the time - relieved by eating 🙄

I'm eating smaller portions than usual just in case.

SandwichesAndGingerBeer · 23/04/2026 18:08

@NinthBestOption oh and the reason I’m doing the last bit on my own is that I’ve a wedding dress fitting and I can’t lose too much weight after so I’ve got to pause until after the wedding before having another crack to lose the final 3-4kg 😆

If only I’d signed up a few months ago!

@NinthBestOption do you happen to know if you can switch while stepping up a dose? Or would it be better to do the step up with Voy and then switch the next time?

I'm aiming for 9.5 stone, so about 1.5 stone loss. 9.5-10 stone is my ideal weight and what I always was before kids/perimenopause, I anticipate that I will probably regain a few pounds so 9.5 should give me a bit of a margin. I also would consider micro dosing long term if I can swing it with a provider.

Just asked a friend who's been on them a while. She said moving up if you're on a low dose is generally expected, they'll ask more questions when you get further up and won't let you skip a level.

NinthBestOption · 23/04/2026 20:22

I am struggling to eat today. Not appetite suppression because I could happily eat a packet of biscuits but my normal meals seem very hard work.

Ive only had 900 calories and 60g of protein. I need to take progesterone tonight so need to stop eating soon too.

Well my run of “I just don’t feel hungry” finally came to an end yesterday 🤣. I wouldn’t say I ate loads but I definitely ate more than I have been and I actually felt hungry. I was also thinking about food a lot too. The scales have gone up one pound (I weigh every day) from yesterday. I’m just seeing it as data collection at the moment and trying not to get weird and emotional about it. I am very early on in my journey - as most of us are, so I’m being kind to
myself whilst I work out what dose best suits me.

I’ve checked Shotsy and I’ve had 0.11 in me yesterday (a mix of my very first dose of 0.25, which floored me, + Saturday’s microdose of 0.06). Pic attached. I think what I need is something in-between, so my next dose will be 0.12 (half of the usual Wegovy starting dose).

I am just slightly unsure when to do it. My two jab days so far have been on Saturday and that’s been ok but this weekend, we have friends over on Sunday for lunch and I would like to have a couple of drinks. I’m thinking a new half dose of Wegovy might not be wise the day before. Or am I overthinking it? My other option is to jab on Monday.

Basically, I want to have the food suppression Monday-Friday, with it wearing off a bit over the weekend (so I can enjoy myself a little). Honestly, that first week I took Wegovy - I could barely eat anything on days 3 and 4 and 5 was a massive struggle. So - based on this I do think Saturday I quite a good day to jab. I’m still experimenting! Maybe I should just do it tomorrow as usual?

Has anyone else changed jab days? What days does everyone else like to jab and why? For those who like to let their hair down over the weekend - what day do you jab?

Morning all.

@ritalaI jabbed on Sunday afternoon as it was my first opportunity after being away and I was desperate to get started! I’ll see how that pans out over time.

ritala · 24/04/2026 08:59

How are you feeling so far @OnTheTop

Really good thanks! I’m eating small amounts every few hours (a bit like managing early pregnancy 😂) and that’s working well. I was worried about symptoms while I was at work as I’m out and about a lot, so not near a loo 😳. But I haven’t had any worries in that respect.

The scales are down, which is really motivating. I’m out for lunch tomorrow so it’ll be interesting to see how that feels in terms of what to choose from the menu etc.

NinthBestOption · 24/04/2026 09:08

I did some research yesterday and found out the half life of wegovy is a little over 7 days. So on my next 0.25 shot I'll have 0.375 in me, then wk3 0.44, wk4 0.47, wk5 0.485. By wk 5 the lowest level just before I jab will be 0.242. Then it's pretty much steady after that.

I hadn't realised this, so even not moving up the doses I'll keep increasing until wk5.

Slept very badly last night, peeing every 5 minutes, still feel a bit nauseous, no appetite suppression and I'm glad we have no chocolate biscuits in! 1kg down - must be water!

jazzflute · 24/04/2026 11:28

I did my first injection of 0.25 Wegovy this morning. Was so anxious but the injection itself was nothing and managed to get through the first couple of hours after without giving myself a panic attack! Will track how things go this week and report back!

That’s interesting!

My scales are down 1.4kg from Monday to Friday this week, I expect a chunk of that is water/inflammation, but I’m going to up my calories as I only want to be doing .75 a week loss.

I’ve been thinking I might not move up from the lowest dose at all, as I just want to keep this steady and manageable, without side effects.

ritala · 24/04/2026 12:47

@OnTheTop - ahh that’s great! Yes, I’ve not eaten out yet either. Let us know how you get on!

@SandwichesAndGingerBeer- I feel
the same, I don’t really want to increase either as I’m really worried about side effects, especially the fractured sleep one. So far on this 1/4 dose, I’ve been ok but it’s basically worn off now and the snack cupboard is calling me…

If you really want it and can’t get it prescribed just lie about your height or weight. I knocked 2 inches off my height to get it. It took me from a bmi of 29 to 30. You might have to knock a few more off or hold a weight while you’re standing on the scales for the picture. And before the holier than thou gang come for me, I’ve lost 3 stone, gone from bmi of 29 to 23, no longer have high cholesterol, am fitter than ever, eating habits are better than ever, massively improved my performance at my sport, which has massively improved my level of happiness. I’m early 40s and had been trying for years to lose this weight and it just would not shift so I have no problem with a bit of exaggeration in order to get something which has literally been life changing and hugely improved my health

TheBroonOneAndTheWhiteOne · 24/04/2026 13:23

With regard to alcohol: I've found ( over the two years I've been on MJ) that I rarely think about alcohol.

If I drink, I definitely get a headache the next day. That didn't used to happen. I never drank a lot, but I generally had one cocktail in the evening before my dinner.

Now, In so used to doing OMAD at about 4pm that the thought of alcohol so early is nonsensical, so I don't drink. Not at that time. If I go out, I do drink, but not much and not always.

The day I've injected has never made any difference to the reaction I've had when drinking alcohol.

TheBroonOneAndTheWhiteOne · 24/04/2026 13:28

Another thing about MJ and alcohol is that there is research being started now to see if alcoholism and tobacco addiction can be treated with MJ.

The treatment for both those serious health problems could hopefully soon be revolutionised.

That's not surprising as your dose is minuscule.

How about treating your SE more vigorously as soon as they appear.

That's where the fat cells go. They shrink and are excreted in your urine.
Well, and you breathe and sweat them out, too.

I like it when I pass a lot of urine on MJ because I know that the scales are about to show a loss.
