I suspect the reality is that the RF really wanted to give Kate full privacy about her health to try to avoid the sort of shitshow that surrounded Diana. Then, despite them being absolutely clear that she was out of commission till Easter people kept speculating so they issued the photo in an attempt to shut everyone up. I can quite believe that they remembered that they usually issue something for Mother's Day so William took a few quick snaps which Kate edited to improve. Really they would have been better off just reminding everyone that they had said she was off till Easter and nothing had changed.

This. I've been thinking for a while she has Crohn's or inflammatory bowel disease. It's the only explanation connecting all the dots. One of my brothers had Crohn's diagnosed when he was 35 and the first few months were a nightmare. He had two surgeries in the first 3 months, developed a lower back fistula which needed another surgery, had a stoma and was so full of corticosteroids his face looked like a full moon.

The problem has been how KP and ultimately William, have handled all this in the public eye. It's been a PR disaster when it could have been handled with more transparency without having to disclose the nature of the medical problem. A few announcements from time to time saying Kate appreciated all the good wishes, and was recuperating well. William answering questions to the public in the same manner instead of giving complete silence. He and Kate have a right to privacy but they also have serious institutional responsibilities to the country and their people. Combining both is the difficult part and frankly, they've made a mess of it.

I'm surprised nobody from the hospital has leaked any details.

I'm outside the UK too but in a country which is friendly towards the royal family and where the media is largely controlled by the same players as it is in the UK. So no surprise it's not mentioned here either.

From what I’ve heard from an old contact Kate had surgery to separate gynae organs fused to colon, and Charles’ cancer is worse than admitted. (Skeptical about the latter as I think if that were true he would be doing surgery/chemo etc).

Someone pointed out that the photo wasn’t photoshopped but was cobbled together from different photos. That makes it a very different issue - proper lying on someone’s part, not necessarily POW’s.

Or perhaps as she was due to have unavoidable downtime after a big operation she might have also had a cosmetic procedure a few weeks after the abdominal surgery (less likely)

Im sure any significant abdominal surgery could leave a person feeling super vulnerable, particularly if bowel related. My other half had stoma surgery last year and it took months for him to gain any kind of confidence back. We had a family wedding 4 weeks after the surgery that he was desperate to attend and it really set his recovery back as he was exhausted for weeks afterwards.

Despite being youngish and very beautiful she is a definitely a bit more jowly these days (as happens to the best of us!). I had thought facelift (with or without the reported abdominal surgery), especially when we saw how puffy she was in the car last week. No judgement - I'd do it if I had all that money and all that scrutiny.

However even that slight glimpse of her profile in the car today suggests she still has a little puff of cheek just above her jaw, so maybe not. Surely it'd be the first thing to be yanked tight in a facelift.

I think it's pretty obvious BP and KP have different offices. KP have cocked up so badly here compared to BP it's obvious. Either William is not lustenjng to advice or he has surrounded homself with incompetents sonthey will do what he says. Either way, the buck for this stops with him.
If Kate has booked herself in for a face-lift as soon as she heard about her films cancer, that family is seriously dysfunctional. The current Queen is 70 years old, the one before thst was in her 90's. Why would Kate with a face-lift make a difference? She had the normal crows feet of a 42 year old woman in that photo.
