I think some people think it's similar to Emma and Caspar but he was clear it was a weight issue with him (despite him being similar sizing as her) and that she was like his sister. She couldn't have been more than a size 16, so not similar to very overweight Dean at all in my opinion. Caspar also was completely blocked off and kept saying he wanted to leave without any real effort for the first few sessions. Sarah has said she had the ick to some girls at a dinner party once and we all see Dean is intensely annoying and manipulative so it's not really comparable.
