5th thread on prepping for a pandemic and following the risk of a COVID pandemic.

Anxiety

As Preppers we tend to look at a reasonable worst case scenario to plan for. Everyone is welcome on the thread but if you find it makes you anxious be kind to yourself and think about sticking to once daily updates from the BBC or similar Thanks

General COVID Info

The estimated replication rate is R= 2 to 4 based on latest expert estimates. This means each infected person spreads the virus to between 2 and 4 people. Experts estimate that, unchecked, it could infect 60% of the population.

Around 15-20% of cases are thought to be severe - that is resulting in the need for hospitalisation. Around 3-5% requiring ventilation.

The estimated mortality rate is around 1-2% at the moment (compared to 0.01-0.1% for flu). This may change as it is very difficult to estimate mortality.

Children tend to have milder symptoms. Those over 60 with underlying health issues and a history of smoking are more likely to be severe (although not exclusively this type of person).

Spread of COVID

It is estimated that the average incubation period is 3-5 days but can be as long as 24 days.

It can be spread with no symptoms.

It can be spread via droplet (cough/sneeze), aerosol (breathing same air in very close quarters), bodily fluids, fecal/oral route and formites (via surfaces, up to 28 days in the right circumstances but up to 3-5 days in more typical circumstances).

As this is a novel virus and knowledge is constantly being updated - I will post updates as they become available with links to source.

I am not a medical expert so any opinions or conjecture of my own should be taken with a pinch of salt!

Prepping Scenarios

Most of us are prepping for the following situations:

  • Wanting to stay indoors for 2-4 weeks + in case of a local outbreak / schools being closed
  • Potentially being ill with a flu type illness lasting 2-3 weeks with one or more of the household having the illness
  • Having to take a family member to hospital for COVID or any other reason during an outbreak

Prepping Items

Threads 1&2 have lists of things to think about to prep for the first scenario as do the Brexit prepping threads.

Main differences/additions to something like the Brexit list are:

  • Face masks if you can get them for any trips you have to make in public during an outbreak. These don't offer full protection and cannot be 100% relied upon but are probably better than nothing if you make sure you wear them correctly and dispose of them correctly. N95 masks are best but expensive and harder to wear, surgical face masks are second best. Dust masks are not going to help.
  • Hand sanitiser needs to be 60% or more alcohol content
  • Dettol / bleach / Miltons to disinfect. Not all disinfectants work but these three do
  • Plenty of at home/over counter treatments for flu type symptoms should you need to treat at home
  • Tissues (lots of) should you catch the virus
  • Think about meals that are easy to cook and eat should you be ill or both parents be ill at the same time (soup for example)
  • For a potential hospital trip you may want to prep a 'go bag' should you need to take someone to hospital with the virus (or anything else) during an outbreak. See Thread 1&2 for examples of what people are packing
  • Frequent and thorough hand washing, not touching your face and social distancing (no handshakes, hugs, etc) are some of the most effective ways to stay virus free

Other Thoughts

India have announced that they are stopping exports of some antibiotics, hormone treatments and vitamin supplements as they rely on China for raw materials.

Many factories that supplied fibre for use in sanitary towels and tampons are being diverted to manufacture face masks. Consider stocking up or changing to sustainable items (cloth STs, mooncup, period pants)

I wonder will the coach driver etc meeting the Diamond Princess evacuees wear PPE? I know they haven't the last (3?) times and I guess that hasn't proven to be a problem since all those people ultimately tested negative but surely this time there is a high risk that one or more of the passengers will be positive and therefore masks and gloves would be sensible?

Korean cases have taken off, though a lot of them are linked to specific places.

I think PHE should issue warnings for travel to Italy and Iran as well as Asia. They were only warning for Hubei a long time after it had spread to other areas of China.

The response to the virus needs to be quick to adapt to new information. It seems too slow here.

The Lancet are reporting that those that die tend to have been in ICU for 1-2 weeks prior to death which is a hell of a thing for the health services to try to deal with

Of 710 patients with SARS-CoV-2 pneumonia, 52 critically ill adult patients were included. 61.5% of those critically ill, died.

The mean age of the 52 patients was 59·7 (SD 13·3) years, 35 (67%) were men, 21 (40%) had chronic illness, 51 (98%) had fever.

32 (61·5%) patients had died at 28 days, and the median duration from admission to the intensive care unit (ICU) to death was 7 (IQR 3–11) days for non-survivors.

Compared with survivors, non-survivors were older (64·6 years [11·2] vs 51·9 years [12·9]), more likely to develop ARDS (26 [81%] patients vs 9 [45%] patients), and more likely to receive mechanical ventilation (30 [94%] patients vs 7 [35%] patients), either invasively or non-invasively.

Most patients had organ function damage, including 35 (67%) with ARDS, 15 (29%) with acute kidney injury, 12 (23%) with cardiac injury, 15 (29%) with liver dysfunction, and one (2%) with pneumothorax. 37
(71%) patients required mechanical ventilation. Hospital-acquired infection occurred in seven (13·5%) patients.

It gets worse . . .

The mortality of critically ill patients with SARS-CoV-2 pneumonia is considerable. The survival time of the non-survivors is likely to be within 1–2 weeks after ICU admission.

Older patients (>65 years) with comorbidities and ARDS are at increased risk of death.
The severity of SARS-CoV-2 pneumonia poses great strain on critical care resources in hospitals, especially if they are not adequately staffed or resourced.

So to show how quickly everything breaks down, in order to get info on what to do if you want a test on Italy, you must ring a number.

I've been trying for hours. Just get a recorded 'it's busy' message. Told to check the website. That didn't load for a few hours, but now it's loaded, the message is, ring the number.

DS2 is ill. I'm sorely tempted to give him a course of the antibiotics that DS1 had a fortnight ago for pneumonia. I would never normally consider doing this. We have a full course. The hospitals will be manic. Our paediatrician is unavailable. I don't even know how I'd get him to hospital - his oxygen sats drop when he moves from lying down. But lying down he just feels like he has a cold.

We've come to the shops to stock up on fresh food. Will be interesting to see if it's busier than normal.

Hopefully what has happened in Italy and the speed with which it happened will make people in the UK realise this could impact on us. Its quite possible that any of us could get a knock on the door advising that the area is in lockdown. At this stage I'd be very surprised if some of the people in the UK with coughs didn't have it and so its probably just a case of when people start to get tested and diagnosed.

We have two weeks worth of stuff. DH is in town and so I'm asking him to pick up some more cold and flu meds and animal feed on the way back. That's it then

Also trying to look on the positive side and thinking of all the DIY we could get done if we were physically well but in two week lockdown!

That must be so worrying Veneto resident hope your DS starts to feel better soon. I can see where you are coming from with the thoughts about the antibiotics, do you have any friends with medical training who might be able to advice you? Guessing it's nearly impossible to get to an actual doctor right now. Must be so surreal as well with the lock down. Hope you manage to stock up at the shops.

This thread has been really useful for me and I have started to get some bits together in case we are told to stay in.

It really worries me how complacent some people are and I fear these will be the ones that will spread the virus.

I have been shopping and it doesn't seem busier than normal. I honestly think people are unaware and it's only a small minority prepping

@CrapTVAddict I agree. I'm just going shopping now so not sure how busy it'll be but my partner thinks it's funny I'm preparing and "it's just a cold". This will be the attitude of the majority of people. I'd rather be prepared than not!

We are absolutely broke and have used some of our preps. We get paid on Monday so we will go shopping then.

DD in hospital after emergency surgery. We don't live in the city, and didn't realize we were overlooking the infectious diseases unit last night!

Hasa that's a great point. Might make sure my DIY supplies are up together too so that if we were to get stuck indoors but be fit and well I could use the time productively.

I'm stuck in bed atm with a flare up of a chronic health problem that I have. Just waiting for meds to kick in so I can crack on with decluttering. I figured if we were to get sick that having a clean and well organized house would make life a lot easier. In fact I feel like that today, I feel rotten and the last thing I want to.do is declutter so I think it's a must now!

Per the Guardian the bus driver taking the DP passengers to the Wirral is NOT wearing protective gear! I find that shocking as there is a significant chance at least one of them is incubating the virus.

Mini update:

  • 2 new cases in UAE from Iran
  • It has been found that 9 people from Korea who have CV took a trip to Israel including all of the most crowded tourist/religious spots so that could cause a problem
  • Italy now have 54 cases(!)
  • South Korea now have 443 cases(!)
