What is a pelvic prolapse?

As the muscles, ligaments and supporting tissues in the pelvis become weaker, they are less able to hold in the organs of the pelvis such as the womb (uterus) or bladder.

Gravity pulls these organs down and, in the more severe cases, may appear through the entrance to the vagina.

A variety of problems can occur, depending on where the weakness lies and which organs are able to descend, but in every case there is some degree of prolapse of the vaginal wall, which begins to invert (rather like a sock turning inside out).
Prolapse of the womb or uterus is the most common prolapse, affecting as many as one in eight older women to some degree
Prolapse of the bladder, known as a cystocele, is less common.
Prolapse of the urethra (the tube that carries urine out of the bladder) is known as a urethrocele.
Prolapse of the intestines is quite rare, and known as an enterocele or rectocele.

Symptoms

Symptoms depend on which tissues descend, and how severe the prolapse is.

They may include:
A sense of heaviness or pressure in the pelvis.
The appearance of a bulge of tissue in the genital area, which can be quite alarming, and is often red and sore.
Urinary problems, such as having to urinate more frequently, feeling the need urgently, being incontinent (losing control of the bladder) or, conversely, being unable to pass urine when you need to.
Pain in the pelvis or lower back.
Sexual problems, including pain and decreased libido.
Constipation.
Vaginal discharge or bleeding.

Treatment and recovery

Once a prolapse has developed, surgery to fix the affected organs is usually the only way to cure it effectively.

However, another option is to use a device known as a vaginal ring pessary. This is rather like a contraceptive diaphragm or cervical cap. It's made of silicone or latex, and placed in the vagina to push back the prolapsed organs and hold them in place. Many women happily manage their prolapse this way.

Wouldrather and Skinikki - I can recommend a very good instructor at Big White in Canada and Perisher Blue in Australia if you go there (my brother)! Have to say too energetic for me tho! Smile

Would love to ski over there. Have only skied Canada a couple of times and had 60 kids in tow so not much of a holiday!!!

I'm also thinking about skiing at Christmas, but am so nervous about new scar tissue that I keep delaying the booking! Can't decide .. really don't want to go through this all again (plus I probably don't have enough tissue left to use in more repairs!)

Fen - regarding time between ops, I was supposed to have anterior and posterior repair Nov 3rd, but the consultant found a 'large' enterocele which entailed slicing all the way up to the top of the vagina and I think he ran out of time for the anterior, plus he had me down as day surgery!! Hmm. I then had a lot of pain for 3 months and the consultant kept making excuses, didn't want to 'have a look' (in case he disturbed things) and then booked me in to examine me under ga (Jan 15th) but cancelled on the day (after I was already gowned and stockinged). So, thanks to my mother, found another consultant who over the phone suggested that I probably had the wrong sutures in which wouldn't dissolve. Therefore, surgery Feb 3rd, opening up top layer (vaginal tissue, not fascia underneath) to remove 7 stitches, and a re-sew. I then had problems with the cystocele getting bigger and progressive incontinence. Next op was June 22nd for anterior, posterior (again - but not all the way up) tvt insertion and perineum repair. I was surprised that the second consultant wanted to do the posterior again but he said that he always did both together to restore correct anatomy (and I suppose otherwise there would be a danger of pulling on recently formed scar tissue). He did, however find a small rectocele, so lucky he did. I now have a very tight fanjo (barely a finger!!! - but there is still swelling! He examined me on Monday, but only fingers, no speculum) - will have to go to Iceland for new partner, or Asia! Lol!!! Wink. But I really don't care as it will all eventually stretch again to a 'normal' width (so I've been told) and I do not have any more bulges (hurrah) Grin. I am only 2 + weeks post op and I can already tell that everything is more or less 'back' to its correct position. Didn't have this feeling before. My nice consultant didn't seem at all concerned that I was being opened up for the third time in 6/7 months.

Sure - not long winded at all. Thank goodness you finally found such a good consultant and are finally able to look towards a full recovery. Oh to have everyting in place and all neat and tidy!!!!!

Fen - just imagine .... adventures of the fallen fanjo club in Whistler !!!

Skinikki - it is a small world indeed. I have met them but don't really know them as used to go on a Friday night, plus know peeps from the Thursday day class. It's great to find some kindred spirits on here as it can sound a bit fickle to say I haven't been able to ski but it is a bit of an obsession with me and look forward all year to my week on snow. I am so relieved to be able to walk a fair distance now but for months running up to my op in March I was so uncomfortable I just about dragged myself through my day at work to crawl onto the sofa for the evening. Emotionally these conditions take their toll. Thank you so much for sharing the positive news from your consultant - it gives me hope but I also feel a bit like Sure in that I'm worried about undoing things once I'm finally repaired.

Sure thank you for the info. All useful stuff especially as I am getting more frustrated with it all. Still have a large 'lump' just inside at the front (after ten weeks can't still be swelling) which if I wanted to, would make sex v difficult. Have what I can only describe as a sea urchin at the back which hangs out. All together with occasional pain round my backside and more often around the back part of my fanjo which radiates to my buttock and down my leg. As for bm's, the u-bend has gone but I'm
'going' 3 or 4 times a day which I didn't expect and still having to 'double-void' when my bladders full and feeling the need to go far too often in my opinion.
Sorry for the huge moan... Haven't said it before on here but I really do feel I'm the one who's suffering due to poor management with my life yet again on hold. And as for sex, i wouldn't want to let anyone near me, not cos of the healing but because I don't look anywhere near normal and can't imagine anyone getting past the lump. Must stop feeling sorry for myself as I can't afford it!! Bought a new sofa on Sunday partly because I was feeling down and mustn't spend any more (have already had to replace blown up oven and get heating repaired recently)... I'll shut up now as I'm embarrassing myself and about to cry. Thank goodness I have you lot to talk to as I don't think anyone else really understands and most of the time I laugh it off. I really hope you're feeling better Sure, and looking forward to finding that magic wand. Hugs (((0))) and (((8))) as appropriate to all.

Fen - I can totally empathize with how you are feeling - I was so depressed after my first op and cried myself silly for about a week. I had mentally decided that the new year would herald new job, new house, new man, etc. etc. having spent years only concentrating on my kids and others, and felt that I could get fixed and it would at last be 'me time', only to find that all the pain and trauma had got me absolutely nowhere (and with regard to sex, was too embarassed to admit how long ago that was when asked - too many lumps and bumps for too long [embarassed]) Hang in there girl, you will get sorted but it might take longer than you anticipated ((((big hug)))) Also, end of term syndrome probably, when you are on your knees anyway xxxx

My SIL sent me a get well card, and on the front were two sea urchins, one on top of the other, looking like a prickly number eight. She couldn't understand why I howled with laughter so I had to tell her all about MN and the fallen fanjo club and how I didn't want to be an ((8)) club member anymore Grin - think she thought I was still under the influence of narcotics!! Lol!

Should we organise a 'FF' club skiing party???

Seems a lot of ladies on here have had Rectocele repair then had to have cystocele or vice versa, would like to here the circumstances if you don't mind....still can't believe I've got to go though another Op...I don't know how long I can stand these pressure pains & wanting the loo all the time..Dr said he can give me tablets to take those feelings away, any one had an experience of these?

Hi Gail - my consultant advised me to stay away from the tablets as the side effects are not pleasant (didn't question him as I don't like taking tablets anyway) but this was after urodynamics study looking into stress incontincence when we were discussing tvt op.

It seems to be that only urogynaecologists will do both ops in one as most gynaecologists seem bladder shy (!?) - just a thought, as my (nice) consultant didn't bat an eyelid about doing everything in one go and he is a urogynae. The GP's either don't know or simply don't refer us to one as they are hard to find!!!

Fed up today still got my cold and I am a bit achy down below but do not think there is any damage. I think that is the real problem the feeling of being worried about my own body. I wonder when I will ever have that confidence again.

Fen it good to get the feelings out and we are all here for you. I think you are a strong women and you will get through this. I also have a small lump near the entrance I am seeing my consultant next week and it is the first this I will ask about.

Sure so glad you are happy having read your story on this thread for so long it is great that it has finally turned around for you.

I have not skied in years and all this talk of skiing is making me nostalgic. :)

Hi ladies, hope you're all well.

Looking for any advice pls - period started about lunchtime today and am already in a lot of pain in the rectal area (guess around the site of the repair). Does anyone have any suggestions as to how to calm it down?

Sorry to whine but am in agony :(

RoseAnna I know how you feel it awful isn't it. I found an ant inflammatory and a hot wheat bag helped me. I also had a day in bed :(

Thanks Pip. Was taken by surprise a little as I hadn't considered it at all. I've taken the maximum painkillers I was prescribed when I left the hospital (2 x 500mg paracetamol and 1 x 500mg ibuprofen). Will send out DH to source a hot wheat bag...can't wait to see what he comes home with tho lol.

Do you think I might be ok to try a bath or will I dissolve the stitches? Am 23 days post op.

Hi Roseanna - got my period today too :( Not too painful yet but have a feeling tomorrow will be ghastly, if my experience after last op is anything to go by; I took to my bed with two hot water bottles, front and back, and lots of painkillers!!! (and I don't usually have much pain other than a dull backache for a couple of days). You should be ok to have a bath as my GP told me to have sitz baths for the infection and said stitches would be fine if I wasn't in too long, but I wouldn't add any bubbles xxx

I have been having baths and I am full of them! So I don't think baths dissolves the stitches. I would have a bath if it make you feel better. I just make sure I do not stay in too long.

Ladies it is so good to read all your posts as most questions are answered at some point and if they haven't been someone will know the answer. I am amazed at the strength and determination of people on this thread, imagine if men had these problems........we wouldn't hear the last of it! I am glad to hear that some of you are getting sorted (eventually) and recovering even if it is slowly! For the skiers out there I think we should all book our trips to give us something to look forward to/ aim for :-)
I actually got out of bed today, got dressed and up thinking I felt better. I have actually just spent the day on the sofa with a constant dull ache and feel knackered.......how? I haven't done anything!!! Looking forward to going back to bed!!

Skinikki - you're right, the ladies on here are just amazing and I don't know what I would've done without their advice and support. One of the great bits of advice I got was not to go getting dressed too early though because if you stay in your pjs noone expects you to do things :o

Skinikki - all quite normal! I was getting up and kind of dressed and sending dd2 off to school - and then needed a sleep! I had a hysterectomy (sorry can't remember what you had done) but found I had a period type pain but more spread out iyswim? They really do pull you about during these ops so you're not only healing but bruising swelling and annoyed nerves are all settling too. Just keep taking it easy and listen to your body! Smile

Roseanna - I shouldn't say it (!) but I've always found a sherry, whisky or brandy are good for period pain - definitely not more 'dilute' drink! Grin

Funny you say that Fen...DH just poured me a rather large nightcap. Probably shouldn't after all the pills I've had, but its certainly making me feel a lot better :o
