What is a pelvic prolapse?

As the muscles, ligaments and supporting tissues in the pelvis become weaker, they are less able to hold in the organs of the pelvis such as the womb (uterus) or bladder.

Gravity pulls these organs down and, in the more severe cases, may appear through the entrance to the vagina.

A variety of problems can occur, depending on where the weakness lies and which organs are able to descend, but in every case there is some degree of prolapse of the vaginal wall, which begins to invert (rather like a sock turning inside out).
Prolapse of the womb or uterus is the most common prolapse, affecting as many as one in eight older women to some degree
Prolapse of the bladder, known as a cystocele, is less common.
Prolapse of the urethra (the tube that carries urine out of the bladder) is known as a urethrocele.
Prolapse of the intestines is quite rare, and known as an enterocele or rectocele.

Symptoms

Symptoms depend on which tissues descend, and how severe the prolapse is.

They may include:
A sense of heaviness or pressure in the pelvis.
The appearance of a bulge of tissue in the genital area, which can be quite alarming, and is often red and sore.
Urinary problems, such as having to urinate more frequently, feeling the need urgently, being incontinent (losing control of the bladder) or, conversely, being unable to pass urine when you need to.
Pain in the pelvis or lower back.
Sexual problems, including pain and decreased libido.
Constipation.
Vaginal discharge or bleeding.

Treatment and recovery

Once a prolapse has developed, surgery to fix the affected organs is usually the only way to cure it effectively.

However, another option is to use a device known as a vaginal ring pessary. This is rather like a contraceptive diaphragm or cervical cap. It's made of silicone or latex, and placed in the vagina to push back the prolapsed organs and hold them in place. Many women happily manage their prolapse this way.

Hi to all those only being offered a rectocele repair - quite a few have found after a posterior repair that the cystocele gets worse as the rectocele held the other up. My original surgeon didn't touch the cytstocele (because he ran out of time....) and now I am having to have the cystocele done and the rectocele done again because apparently to do it properly, it is better to both at the same time. So, ramble over, I would defo get second opinions and ask why they don't want to do both in the one op. Once the vagina is weak and stretched then no amount of pfe can help, so the cystocele will only get worse over time, so I was told. Hope this helps xxx

Jackie - I am feeling very unsure about the TVT although the uro/gynae is an expert in this and assures me he is very good at it. I am going to ask on the day if fixing the cystocele will get rid of stress incontinence and the tvt won't be necessary. Has anyone else found incontinence issues better after a cystocele repair??? Are there any women out there who have a tvt and are really happy with the result???

Sorry to come back here with another question.... I've now had three days of new ab's, still feel awful (don't think I have a temp tho) and no change in symptoms other than more blood-stained now. Swab results not back. Do I sit it out or go back to gp? I don't fancy the idea of waiting over the bank holidays to see gp if still not right, or seeing emergency doctors as they should have taken swab originally and didn't. Wish I had something more cheerful to say and feel like I'm turning into a right moaner.....

Should add I've never had a temp, but have been feeling nauseous and dizzy at times since thurs if this is relevant....

Poor you Fen - do you know where exactly the infection is? How long now since your op? When the stitches come away it can cause slight bleeding, quite red too. Really feel for you, big hugs, xxx I would definitely see someone before the weekend, maybe even the consultant who did the op? You could give his sec a ring and say it's urgent???

Sure - I think it's in the wounds. Am at a loss as to what to do as hosp could have seen me today but I couldn't go, and can't seer before thurs now. Drs didn't seem that bothered (saw a nurse) but she didn't ask me how I was feeling and I didn't think to say - was too mortified at the appalling smell from the infection. Seem to be getting more and more sore on the outside too. Hope you're getting along ok in the circumstances. X

Oh and four weeks today since op. I think infection has been there apx three weeks but I wasn't sure at first.

Fen I would go to the GP again today and demand to be seen.

Those anti-b's are obvsiouly not doing the job. You must have a pretty horrid infection. I had no smell whatsoever just loads of pain and horrendous bleeding but the anti-b's had definitely kicked in after the first couple of days. The anti-b's did make me feel horrendous as well as the infection doing the same to me. At the very least you need some decent pain killers. The cultures may well be back today as it will have been a week.

Tink I definitely don't regret the operation even having had a nasty infection. My quality of life now is so much better it is unbelievable. No using your fingers to poo which upset me every time, no heavy feeling, tiredness has gone.

Hope you all have an event free weekend. We have my brother's wedding tomorrow which should be fun :)

Have now got penicillin and metronidazole
Gel as well. Have spoken to consultant and I have to go I a&e on mon if no better (which I sincerely hope I am)!

Oh good. I'm pleased that you've spoken to him as well. I do hope that it clears up but make sure that you do go to A&E if necessary even though it's a BH.

Ben10 - I really felt I needed to as the dr said she was concerned about my pain but didn't suggest anything and only gave me the gel after I raised the fact that I still was positive after metronidazole tablets for a week. It really made me feel I was being fobbed off. I'm still waiting for a call back because I phoned to ask if it's strep a or b as the implications could be quite different if the ab's don't work.

Goodness Fengirl - you've really been through the mill. Don't be scared to go to a&e if you need to - my sister works in a&e and from the stories she tells they're often glad to see patients who are genuinely sick on bh's rather than drunk!

An update from me - went to gp again today and saw a different doctor because things seemed to be getting worse. He looked at the notes from the last doc that said no prolapse just 'vaginal wall laxity' and shook his head. He agrees with physio that things are not going to be resolved via that avenue and is referring me to urogynae consultant next Thursday.

Thought I would be upset, but feel strangely relieved to get a specialist opinion and guess I'll know more about exactly what's wrong. Can anyone tell me if he'll be able to tell me what's wrong and possible options at the first consult or will I need more tests etc first? Sorry in advance if thats s really stupid question!

Hi roseanna - the consultant should be able to tell straightaway what the problem is as this is what they 'do' for a living and see it all the time. Make sure they examine you standing up and coughing as sometimes everything can disappear when you are lying down. If you have incontinence they will suggest you have a urodynamics test done (lots of weeing in front of people, lol!) but nothing too dreadful. Keep us posted xxx

Fen - I hope you begin to feel a bit better. Please go straight to a&e if you aren't feeling any different over the next couple of days.

Pip - wishing you lots of luck. Take some music with you as you won't feel up to doing much else, and it is quite comforting. I borrowed my son's ipod, unfortunately he only had those massive earphones which did look a bit ridiculous, but hey... who cares, lol! xxx

I have been reading medical documents about prolapse etc and apparently there is some link with vitamin B12 and zinc deficiencies and prolapse so I have been to the health food shop today to get both and see if improves any of the symptoms before my op. It also said something about a genetic link with inability to absorb these vitamins from food ... ???? Worth a try...x

Sure - I certainly will - especially as I've found out (not that the dr bothered) that the strep is quite an unusual one and can be very nasty.

Fengirl - hope sou start seeing an improvement soon. How's your mum now?

Mine's feeling worse today, I spent virtually the whole of Wednesday sat in upright chairs (full day at the office vs my usual 3 hours then straight out to pub and restaurant). Today I have been on my feet a lot, I feel like I did a week after giving birth, all uncomfy and achey. I'm glad my gynae appt is next week.

I really want to go and lie down now, so no more chatting but I would just like to say that it is brilliant knowing you lot are all here knowing exactly how this feels and willing to lend a virtual ear.

Whoknows - mum is getting there, but slowly which is trying her patience! Poor thing has 31 clips and a 12 inch scar.
It's not at all unusual to feel much worse if you have over-done things. I found I had to completely change the way I worked pre-op as I was usually on my feet a lot. Hopefully it will settle a little bit but it also gives you an idea of how things would be ALL of the time if they deteriorate.... Not to upset you but to make you aware. I was stupid enough to do a couple of things pre-op that made the pain much worse and I really regretted it!

Hi ladies! Just checking in to say I'm still a bit tired but feeling loads better (thank goodness!) and even managed to go to see DM today who is looking a bit drawn but doing well. Feeling it a bit now after an hour's drive each way tho! Hope you're all having a good weekend. Smile
