Deteriorating again in hospital

Argh.
Mum was ok on fri having gone in xmas eve. Plans afoot to get her back on wafarin after the weeken and home ASAP.

Yesterday coughing lots and mysteriously on oxygen. Sta,paed my foot a bit and insisted on seeing a doc. What point is " they'll be here Monday " ? No that not ok. She's not okNOW ! Anyway. Seen and xrayed " don't think its infection ".... Yeah right

Today she's much worse. Away with the faries ( not her at all) scrabbling at the bedding etc. Had oral antibiotic at 3pm but thatsnt going to work fast enough is it? Awaiting more bloods and another doc review.

Darent leave. :(

Have you told them you are a Dr? Time to pull rank and be the annoying medical relative I am afraid. Shouldn't have to do it but I have found it is effective when advocating for my terminally ill mum.

Be assertive and ask to speak to the on call Dr yourself or to escalate to the reg or consultant if they are not willing. Present her symptoms in a detached professional way (I have been known to quote NICE guidance)

She's sat out now and clearly ventilating better with oxygen via mask rather than spec as was mouth breathing and nose bleeding !

Hope they'll be here soon it has been promised.

Hope you get seen soon, I was amazed at how quickly my mum responded to iv antibiotic so please push for them. Also make sure they know she was mentally fine at Christmas.

She was mentally fine on Friday / Saturday even!
Bloody hell if it was 54 mins at 2205 it's now 84 mins!

Grand now the nurses don't have time to call. There is casually nothing active going on but I'll give them 2 more mins and call myself via switch I think!

New onset confusion with the Obs you've said are raised suggests she's scoring enough for a sepsis care bundle to be started. Ask the nurses if it's been done and if not should it have been according to their policy?

Resps are24-28 depending if awake or asleep. She's had s drink and looks a bit better say out and on a decent O2 amount via mask.
Sat out in chair and seems easing breathing
Obs have been some and aren't changing.
Have resorted to calling the senior sister myself via outsiide line as ward clear think I'm being too much of a pest !

Just do whatever it takes. Call switchboard and say it's Dr so and so they will assume you are a GP.

I have called the consultant via switchboard before now when my mum was very ill and not getting good care. He was not best pleased but I did not care at all. I never had to meet him again!

I am not normally a person to do that and nor do I think it is right that 'he who shouts loudest' but when it comes to my own family I was no way going to stand by and let them screw up.

Start mentioning unacceptable delay and that you are going to complain that will likely get some action.

Hi Thea. Her resp rate is concerning. I would have a look at the ons chart and calculate her News score. Ask what the hospital policy is for escalating g given her News score or Early warning score. Ask to speak to the site manager. Threaten a formal complaint.

Do not phone the cons directly they would think you were very ott. They may not be on call and it would be unreasonable.

Good thought. Usually of pts have a v high resp rate the protocol is for register to review and outreach to be called.

Well finally seen by a pleasant ( if slightly smarmy) registrar who " appreciated all my concerns" and has agreed to iv rx which is really all I was concerned about.

At least he didn't try to bs about the bits like the clotting that he didn't understand so kudos for that Mr registrar!
