If I interpreted the Lancet article you linked on the last thread correctly (not an expert, may have misinterpreted) the discussions about rationing access to invasive intubation depending on age/underlying health problems look fairly academic, as the survival rate in the study of people who underwent that procedure was tiny. I don’t know enough about ICU to see if the same applies to other IC interventions.

So we are going down the route of self isolate at home upon showing symptoms for 7 days and not to notify or ask for a test from 111 unless symptoms continue past 7 days. Can the government give us assurances that this method will apply to all sections of our population for example high profile sports players, politicians and their families, TV and film personalities, super wealthy. Because at the moment it looks to be one rule for the 'herd' and another for the special people. If this is how our country is going to tackle this virus then it should be special treatment for our emergency services, medical staff and essential services employees only.

I get you, Cheeseismylife, I’ve been thinking the same but it involves brick dust and I haven’t a hope in hell’s chance of getting a mask from anywhere!
Sainsbury’s had lots of the vinyl disposables in last week, which surprised me. May well have gone now but agree some might be lurking in B&Q etc.

So all football cancelled, some live TV shows cancelled, but all schools,colleges are open.
A footballer has covid19 and self isolates with his team,a child has covid19 and the school opens as usual.
Just dont get it 🤷‍♀️

@MurrayTheMonk I'm not sure that diy style gloves would be great as PPE would they? The ones I normally use as really thin.

@Saucery You might get masks from a diy shed, I think I read that the more basic diy ones are no good as ppe, not good enough filter or something.

DH uses those disposable gloves at work so they have a large stock. If things get as bad as we seem to think, I will suggest he donates them to the local hospital, if they want them. I do have a box and a half here, but that's kind of historic and I can't imagine they'd be particularly useful sterile wise after where they've been stored - but then I'll need those if we get hit.

This way you don't take PPE from people who need it and you aren't throwing away gloves needlessly?? Seems like a no brainer Hmm

No they aren't ideal for care purposes. But none of my usual suppliers have any gloves or masks left currently-none in for three weeks. I've got three boxes from a chemists which will last two days at work (and only for staff with medium sized hands). After that god knows 🤷🏽‍♀️
If I ever find out who nicked four big boxes from our storage shed they can bloody well get ready!

Washington state is the worst affected in the US. This is from a front-line ICU physician in a Seattle hospital.

  • we have 21 pts and 11 deaths since 2/28.
  • we are seeing pts who are young (20s), fit, no comorbidities, critically ill. It does happen.
  • Currently, all of ICU is for critically ill COVIDs, all of floor medsurg for stable COVIDs and EOL [end of life] care, half of PCU, half of ER. New resp-sx pts [respiratory symptoms patients] in Pulmonary Clinic as offshoot
  • We ran out of N95s masks (please stop hoarding!) and are bleaching and re-using PAPRs, which is not the manufacturer's recommendation
-terminal cleans (inc UV light) for ER COVID rooms are taking forever Enviro Services is overwhelmed. Bad as pts are stuck coughing in the waiting room. Rec planning now for Enviro upstaffing, or having a plan for sick pts to wait in their cars (that is not legal here, sadly)
  • when resp failure occurs, it is RAPID (likely 7-10d out from sx [symptom] onset, but rapid progression from hospital admit). - Common scenario for our pts is, admit 1L NC [oxygen via nasal cannula]. Next 12hrs -> NPPV [non-invasive ventilation]. Next 12-24hrs -> vent/proned/Flolan.
-Given the inevitable rapid progression to ETT [endotracheal intubation] once resp decompensation begins, we and other hosps, including Wuhan, are doing early intubation. Facemask is fine, but if needing HFNC [high flow oxygen via nasal cannula] or NPPV just tube them. Will need a tube anyway -[Multiple patients have had normal cardiac function, then develop horrible respiratory failure, then with treatment seem to turn the corner, come off the ventilator, tube out, looking good. Then within 12 hours go into cold clamped shock that looks cardiogenic, cardiac function drops to 10% or less, go into cardiac arrest and die, all in less than a day]
  • Needless to say this is awful for families who had started to have hope. - We have actually had more asystole than VT, other facilities report more VT/VF, but same time course, a few days or a week after admit, around the time they're turning the corner.
