To ask advice on this foot wound? (Pic included)

So last Friday (10 days ago) I fell off a children's roundabout at work and got a few cuts.

This one on my foot was cleaned straight away and I've been cleaning it with TCP but it's getting worse, my whole foot feels bruised and like the skin is being stretched tight and hot to touch too. My lower leg aches.

I plan on seeing pharmacist tomorrow but noticed there's some red lines coming from the wound and when touched it's very tender, I've a few chronic health problems, not too worried as temp is 37.4 it's just so painful.

Anyone got any tips? Don't want to consult dr google and can't sleep with the pain. Feeling such a wuss.

Ouch OP, that looks bloody sore.

Keep an eye on your OBs, maybe write them down somewhere visable in case you need to give them to someone. If you get sepsis symptoms then ring 111, or even for an ambulance. Are you able to do a tiny bit of emergency training with your dcs? So they know how to call an ambulance if you can't? So 'Oh mummys poorly foot has reminded me, this is what we do if we find someone really poorly and they can't talk themselves, we unlock mummy's phone like this, we live at this address, and we ring 999 and tell them our names'etc etc etc.

Maybe also pin your mums number (or any other family/friends/dcs dad somewhere obvious, and set an ICE number on your phone.

Probably all unnecessary but at least you are prepared for emergencies.

Thank you all good tips. I'm immunosuppressed (medication I take for lupus) so had sepsis unfortunately a few times so realise how quick it can happen so just a bit worried as I've had sepsis from something a lot "smaller" but I'm being a wimp with the pain.

Hopefully the antibiotics will kick in soon but if it gets worse then go back to hospital. If you can’t get a taxi then I’m sure an ambulance would get to you and bring you in. Not sure if they’ll take the kids with you so I suppose have an action plan in place

Keep on at them - did you state 'I'm immunosuppressed from x medication' and waving your medication booklet/card/regular blood test records at them every time they looked at you?

I found that got me seen and treated for everything far more quickly and thoroughly than not mentioning it, or just telling one person who probably didn't have time to write it down or the next one didn't read it.

Changed GPs since then and they've told me on the first sniffle or a slightly infected scratch/bugbite on my new medication to 'tell work you're going to the doctors, get a cab here and sit down. We WILL see you, even if there isn't an appointment for a fortnight'.

TBH, I'd probably let/ask them to clean it without anaesthesia as well (I'm not allergic, but I don't respond to the standard local).

And contact your clinic nurse, as it's likely they'll want you to stop taking your DMARDs/etc until it's healed.

@MitziK that's good advice thank you, I've left a message on the rheumatology helpline but the nurse has left the job.
I wasn't even asked my medical history or medication but they were rushed off their feet and I think they probably saw the "previous admissions" number and thought I was a hypochondriac (can you tell I worry what people think?!)
I have methotrexate injection weekly at home so I'm not due my next one until Friday & ive just remembered I'm at the hospital tomorrow for some form of complex long MRI.

Are you able to go back? Really they should have given you IV antibiotics!

Please read this OP - Really don't want to scare you OP but with a doctor mate now (London A&E based so she's seen everything!) and she says GO TO A & E "FUCKING NOW"

OP there's being keep calm and carry on then there's being bloody minded. Tough love time - find a way to get there even if it's with kids in tow.

This is a prime example of what A&E exists for - please go or I'm going to have palpitations about it and have to go myself!!

Been there and done that with cellulitis, multiple times. I think you need some heavy duty iv antibiotics.

and when I had it less that that I was on two types of antibiotics and I dont have lupus please go back

You've been asked to go back if it worsens and it is. It's serious. If you can't afford a taxi and there's no buses now (none here after 6) ring for an ambulance!

You need to go back now. The tracking should not be progressing further up your leg and it is fast becoming an emergency. If you can't get a cab call 101 and they will likely send an ambulance.

You may not have a fever because you are immune suppressed, please at least call 111 as you need to be treated properly.

You need to go back, that’s tracking, and I’m guessing it’s further by the time you reply to this. Hope you can get someone to take you/mine the kids etc- I’m the wrong side of Wales to help, or I would!

Good point about the fever there. A fever is a sign of an immune response, yours is suppressed. You cannot take the risk, this can't wait.

You need to be aware that your medication can mask inflammation so it will actually be worse than it looks .

And your Tetnus status ? That wasn't a wound from something clean, most likely a tear or abrasion from some dirty park equipment .

You need to make sure they're aware of your health, the duration and the worsening .And what caused it .

And leave off the TCP , it can cause 'burns' on the skin. Salt water (boiled water with a teaspoon salt and cooled to tepid ) much safer .

Though I have to say the number of times I've sent patients to A&E with an Admission letter and they've been sent home with oral antibiotics ........ I could've got them via the GP !

They don't know you're on MTX. That would have completely changed their outlook - and if it didn't, you should insist that it does.

Get back there now. And start every sentence with 'I'm on Methotrexate and had Sepsis x times...'

You won't be taking this week's dose - or any more until somebody in Rheumatology confirms your bloods are OK to resume - so make sure that you speak to the Consultant/the Consultant's Secretary in the morning anyway.

Agree with the others. I start every sentence with "I'm neutropenic" because otherwise it gets treated like a "normal" patient
