To take Dd to Hospital

So Dd12 had a sore tummy yesterday. It's a band across her stomach from under her tummy button to the top of her thighs.

Last night she did 5 poos and vomited multiple times in the night.

This evening the pain is worse. 9.5/10. Pain killers are making no difference. Spasfon no difference either. She has a temp of 38.7. No vomit and no poo. She feels like she needs to poo. She hasn't eaten all day.

I've rung the French equivalent of 101. They said wait to see Dr. appointment at 11.45 tomorrow.

Tonight is the big student night so hospitals will be full later.

Do I go or not?

I have no medical training whatsoever, but I know when our boy was diagnosed with type 1 diabetes, they commented that if he'd been further along by the time we got his diagnosis and suffering from ketoacidosis, his symptoms would have moved on to severe stomach pain and vomiting. Did they test for ketones? If she's not better by now, I wouldn't hesitate to go back. Hope she's feeling better soon, poor thing Thanks

So we saw the GP today. He said that if no improvement tomorrow then to go straight back to pediatric ER for an ultrasound (if that the thing you have when you are pregnant? Échographie in French ).

Temp is still over 39. Pain is up to 9.5 again. We had a dip to 7 earlier.

When I was driving earlier every bump in the road was agony.

She's eaten a spoon full of rice today.

I'm at a loss what to do to help. Hoping she sleeps some tonight and hospital in the morning I think.

Blood glucose last night was 158 on a very empty stomach with a prick test. BP down to 10/6 today. 12/7 last night.

Oxcat you're right, I misread the 29.0 mmol/mol and thought it was as we have in the UK for mmol/L. That's not a big deal now. I still think that the CRP is very high and I'm surprised that they were content to send home.

Were her blood pressure/heart rate/respiratory rate/O2 levels all ok?

The pain is a 4inch band from tummy button to pelvis left to right. Not worse where you'd expect appendix. .

I’m a GP. I would take her to hospital tonight. That CRP is extremely high, white cells in her wee, and pain with speed bumps.

Textbook appendicitis and she needs seeing by a surgical team.

Hi, If her Pain has increased again I think you should take her back to Hospital.
From what you're describing and her results I'm not sure how Appendicitis has been excluded. Speed Bumps are a Classic sign and the bloods suggest a bacterial infection which could be Appendix rather than Gasttoenteritis.
Hope she's OK.

Guidelines for diabetes diagnosis in children are 200 mg/dL on a random sample or 126 mg/dL on a fasting sample. So something to keep in the back of your mind to ask about.

It seems odd they haven't considered infection/sepsis. Don't hesitate to take her back if concered.

Those levels are only relevant in the US, in Europe and the UK we use mmol/l not mg/dL and the numbers are very different.

OP I really don't think diabetes is likely relevant here, the glucose was only a tiny bit high and that's a normal response to the stress of an infection.

The CRP is very high so she does likely have some kind of infection and the pain being worse on speed bumps does make me think appendicitis, although the white blood cells in the urine could also suggest a urinary tract infection.

I agree that taking her back to hospital is a good idea if she is still very unwell.

My youngest grandson appendicitis started in his back. He had no pain at the front until 24 hours had passed. His was a leaking appendix. Had keyhole surgery everything fine.
2 months later our eldest grandson started with pain in his side just under his ribs, hospital took bloods and said it's a slight infection, only on our insistence was a scan given
his appendix had burst and it had an abscess. He nearly died.
Please take your DD back to hospital and try and get a scan.

Sorry, but she needs to go back to hospital tonight. I am a dr and am shocked that she got home. The pain you describe is classically where appendicitis starts before moving to the right side. The high sugar could also be a stress response. Please take her back tonight.

My SIL had a ruptured appendix this year and was ill for a long time afterwards with numerous complications. If it IS appendicitis, time is of the essence. Hope you get seen again tonight.

A friend's son was extremely ill in hospital and confused the doctors until they discovered his appendix was on the wrong side, so don't let the location of the pain mislead you if everything else points to appendix.

Hope your daughter gets sorted out soon OP.

Please please take her back, it sounds very similar to my "unusual" appendicitis, although my pain was high up, it was no worse in right hand quadrant until much later on... it was misdiagnosed despite 2 nights in hospital on morphine and anti sickness meds, went home poorly but stoic/had been told categorically it was nothing serious... and 10 days later when I was readmitted and it had burst I ended up with a 4 hour op, 20 staple size wound, sepsis and very nearly died, a month in hospital, weeks on numerous IV antibiotics and because I was so unwell my wound opened, took 2 months to heal, I have a massive scar and severe ongoing symptoms, including infertility. Speed bumps hurt, I was so unwell I was delirious at times, and my CRP was high and should have been picked up sooner, as hers should (it's been acknowledged by the NHS 3 years later...) please please go back and bug them to at the very least do an ultrasound that visualises appendix (imperative, they didn't look at mine because it didn't seem relevant as pain site wasn't typical!!!)

Poor child. I don't know the relevance of the test results but I'm astounded they let her home with a pain rating of 9.5!!! I hope she recovers soon.
