to ask if this is an OOH job?

Long story short, months of agonising stomach pain. Back and forth to doctors constantly, told they suspect an ulcer. Been taking tablets so now I get say a week pain free followed by a week in agony.
Doctor told me to book a h.pylori blood test. Can't get through to the phlebotomy service, have been trying for 4 weeks now. Just started a new job and in the middle of training so can't take time off anyway.

Yesterday the pain was so bad all day I felt constantly sick, it was right up in my shoulder as well as my stomach, dizzy, unable to focus, everything. Got worse when I got home from work, couldn't regulate my temperature, had a fever and was in so much pain. My stomach has swollen and it feels like there's a solid lump when im standing up, its literally rock solid. Today I feel marginally better but I'm still stuck in bed, temperature is fairly high and I am in pain. My heart is still skipping beats and jumping all over the place.

I don't know if out of hours could do anything for me. DP is adamant I don't need it but the pain has never left me like this before.

WIBU to go to out of hours? They'll be so busy this weekend :(

Bloody hell, a UTI is agony! I've passed out in pain and also collapsed from one before, no wonder you felt unwell!

I had a kidney infection when I was 17 and thats the only time a UTI ever made me feel so sick and dizzy!

Do chemists open on bank holiday Monday?

Gastritis is vile and has put me in hospital before.

Get onto a very bland diet no onions, garlic, tomatoes, citrus or dairy

I'm a general surgeon. I wouldn't be happy with that diagnosis unless you've had blood tests and an xray. You could have a perforated ulcer, the shoulder tip pain with all your other symptoms is highly suggestive of this.

You could have gastritis and a uti. But really is much more common to have one diagnosis rather than the coincidence of 2 at the same time. You can get changes in your urine suggesting an infection because of external irritation to the bladder (from your bowel contents from the perforation) as well as from inflammation in the bladder from a uti.

If you are feeling better and had an erect chest xray which ruled out a perforation when you saw the nurse practitioner then fair enough. If not, and she just examined you, I'd have a very low threshold to get to a and e.

It might be nothing serious. But your symptoms are concerning.

Thinking about it, and I've had a fair few UTI's, some of them hospital jobs, I've never had one that has given me a solid lump in my stomach or shoulder pain, maybe you should go to A&E anyway, you do sound like you should be seen by a doctor.

Did they do a urine dip to confirm UTI?

Yeah she checked my wee, listened to my chest and stomach, felt my stomach and back and checked my temperature.

Hi OP, another medic here although only a lowly GP not a surgeon. didn't want to read your post & not check if someone has done a pregnancy test. Ectopic pregnancy can cause shoulder pain.

I'm definitely not pregnant, haven't DTD in ages. It had definitely crossed my mind as last time I had a "UTI" i was actually 7 weeks pregnant, but no I'm definitely not pregnant this time.

Pain is ramping up again. I'm going to end up having to go to A&E arent I? :(

Get to a&e before the evening rush starts.

If for no other reason than good pain relief.

Yes, go to A & E. I would not call what you had a decent check up, without a scan. Your case is what A&E is for, you are not wasting their time.

Agree with the docs - I wouldn't dream of giving that 'diagnosis', based on the symptoms/exam you have described here.

What sort of thing should they have done? They were really busy, not sure if that meant they just want to get people in and out quickly?

Go to a and e - you are in pain and sound quite unwell.

As a side issue what on earth does your DP think he;s on telling you whether you need or don;t need to get medical help? Was he expecting you to wait until after the bank holiday weekend to get this sorted, even if it turns out that the UTI diagnosis is correct, in two days time that could be pretty nasty left untreated. Sorry - you don't need to answer any of that his attitude just made me cross, get yourself sorted out pet, and I hope you feel better soon.

I've treated a lot of patients (admittedly mainly kids) with some pretty spectacular UTIs. None of them had shoulder tip pain.... something is irritating your diaphragm, and it sounds like a perforation hasn't been ruled out. A&E before the drunks start arriving!

I know it's a shitty excuse but DD is nearly ready for bed, I don't want to drag her and DP out, I don't drive and the taxis around here are so unreliable I couldn't be sure I'd get home again. If it gets really bad I'll obviously go, but at the minute it's just really sore and uncomfortable.

I've found a pharmacy that will be open Monday so at least I won't have to wait til Tuesday to start antibiotics.

There's a big looong complicated history with DP, long story short his illnesses as a child were always minimised (but of course, if his family got illnesses it was the end of the world), part of a larger scope of his childhood. As a result he minimises his own illnesses massively, but also tends to do it for me too - a coping mechanism I think. When I'm ill, he's fantastic, it's something he's really working on - I think it's just a part of his anxiety.

Blimey Jorah, how many people do you need to tell you that your symptoms could suggest something quite serious? Several Drs and a surgeon have told you to go to a and e. Call a taxi and go!!

You need to go to hospital.
Playing it down won't reduce the seriousness if there's a real problem.
